What form to do for visit?

Not sure what forms to fill out-

Went to do a recert visit today, 4/23. Client's cert is up 4/25. Getting there, suspected client had DVT. Tried to get him into MD office, but they asked for me to send him straight to ER. He is being kept @ hospital. SO- do I transfer him as of today, and let that be my terminal OASIS if he stays past end of cert? I am not sure what type of actual visit I would log on my time, regular SNV, OASIS/recert? So confused- but it seems like I stay this way?! Sure I could ask my boss, but I appreciate input before I approach. :D

I would do a SNV with Transfer to hospital. If his Cert runs out in hospital do a discharge then a SoC on release if he still needs HH.

I disagree. You made the visit in the 5-day window, do the recert if you gathered all the information. Complete the recert paperwork, including orders and goals, and then also complete the transfer to inpatient facility form. When the patient is discharged, he will be a resumption of care instead of a start-of-care. There is no reason to not do a recert.

If you completed the assessment so as able to complete OASIS for recert then, yep, like KateRN1 said - you can recert...then do the transfer not discharged. ROC once home. That is preferable because you are not ending an episode of care - helps agency keep track of quality episodes. If you did NOT get the info needed to complete the recert (like if you sent him out before assessing entirely), then it does have to be a transfer OASIS then a SOC once home.
