What is the chart called ....??

Okay I am having such a brain freeze and can't seem to find it by googling .. the chart where the lab values go .. there is one line then open triangles on the ends containing lab values. I used this in school and can't for the life of me remember what goes in each box .. help anyone???

I've never seen that!

It's good trivia, but to me it's safer not to use them. I know there have been instances where a CBC value has been mixed up with a chemistry value like glucose.
