Seeking a Assignment Worksheet or Form

i am a new lpn and i am seeking some kind of form or worksheet for my patient assignment. i need something to help me get organized on things like: ls, bs, iv, o2 therapy, allergies, etc.

does anyone have such they can send me or share?

we have a 'generic' one at work but its very vague and lacking.

I didn't post this original question but I wanted to thanks everyone for these great resources! They are exactly what I was looking for!
