Report sheet

Hello all! Just had my most exciting thing occur since I've started this job. Student with known, yet undiagnosed, cardiac issues passed out in the boys bathroom. Stated he was seeing stars, BP and HR elevated. RR in the 20's. AAOX3 by the time I got to him. Long story short, no 911 call this go round, just his parents who came to pick him up.

Do y'all have a standard report sheet you use for emergency instances? Something you can provide a copy to the parents for the Dr.? I have one that I made for my hospital shifts, but most of the fields don't apply to the school setting (lab values, protocols, foley placement etc.). I was going to make another one specifically for my school setting but thought I'd see what y'all did!

I know we don't anything here or at least I haven't come across it yet. I was pretty much thrown into the fire and have been running with it - learning along the way. It's a good idea to have one because I know the one kid we did call 911 on - my 2nd day - there was no report sheet but I did give report to the medics. Of course I didn't have anything other than knowing she had asthma and current curcumstances but that was enough.

Good idea but no. I guess because things like that rarely happen. I usually just type something up if needed and print it off but I may make my own form now!

I make a copy of the student's health emergency sheet and on the top, if I get a chance, I will write all vitals, etc I took and hand it to EMS along with a verbal report.

Example: Usually when I have to give Epi, I typically write the time I gave the Epi-pen, where I gave it, and maybe presenting symptoms if I have time. (I don't usually get to vitals in that case unless it is slow allergy development; EMS response here is great.)

Go on a search for school nurse forms, I found lots of non copyrighted forms that I saved for my use. Found a pretty comprehensive 911 report sheet. I can share it if you PM me.
