Question about Grids?

I have heard of them and seen one but don't quite understand them. Is this something that you write on the back of your documentation form? If so, when do you write it? During the planning phase? I'm assuming it is allowed? I would love to see more of them or direct me to someone that can explain it to me. If you have more answers please PM me I would love to talk to someone about it. Thanks so much!

Also I have a 30 minute session with an EC nursing advisor on the phone next week. Is this one of the questions I should ask her? TIA

It's something you write during your planning phase, on the back of your PCS form. It's allowed at that time -- not before, and you won't have time after because once your diagnoses are accepted, you're moving into the implementation phase and it's off to the patient's room! :)
