I work in a different setting(primary care) but I give a limited course of pain medicine and then refer them to Pain Management. This pain management topic is becoming a snowball rolling downhill and gaining momentum.

I work in primary care. A 65 yo man came in with 2 week old acute pain 6/10 to the left groin radiating from the LLQ. He has no significant Hx except for mild HLD, treated with lipitor 20mg (5 years). He denies any lifting or straining precipitating this pain. Some hyper-extension makes the pain worse and the pain is mostly uncomfortable during the day 2/10. A manual exam with cough elicited a positive sign on the left groin and was absent in the right side. He was sent for a Ct abd/pelvis which returned negative findings. What next? Any ideas?

I think it is a benefit to the public. I never knew there was a lack mental health prescribers in my area until I had patients who needed a referral. We are overrun with MD and FNPs, everyone is employed but there is no shortage. I see an opportunity, but not if it requires going back to a 2 year school. Thanks for the info

You are only training and do not count toward staffing....I think they wouldn't want you wasting their time if youaren't going to work there after

I just graduated with my FNP and while in practice I noticed a lack of Psychiatrists and PNPs for mental health in my area of NY. Is there a way to become a Psychiatric NP without going to another 2 year school?

I am new to RAMP and I was at a Peer Group meeting and a discussion started about testing, rules and types of testing. The moderator, an RN and substance abuse phD said that each case is handled differently based on the individual's background. I have no knowledge myself, but she states it is not a "one-size-fits-all" arrangement...of course this may also be not truthful.

I am an RN in NY and have since moved to NJ, in 2011 I was put on probation for not properly documenting wasting of narcotics, never drug tested and no diversion was mentioned. Now when I applied for a NJ license I am placed into RAMP. It has been 5 years since the NY trouble. I just started. I have a case manager. I have no idea what it is I am being monitored for, everyone else in the group are IV drug users or have got caught dirty in a urine test. Some people are being monitored for over two years....really?

I am a frequent visitor to Sandpoint ID, I remember last time they were building a big cardiac facility in Couer D' Alene. Unfortunatelt Sandpoint only has a tiny hospital, which is where I want to move.

Steve I am graduating from a local ADN RN program in May. I live in Rockland NY, is this a place where I could work? I am currently a telemetry/EKG interpreter in a PCU in Westchester.

You have misspelled "pursuing" twice as "pursing." My school was BIG on spelling and would give back all paperwork with spelling errors. They felt that spelling errors on medications could have tragic results. Rockland Community College is a tough nursing school. Of the original 30 students I started with none made it through...and I guess I may be next in my last semester. They let you repeat two different courses only once each and then they kick you out of the program. They do not have an entrance exam so the first classes are loaded with people, mostly non-English speaking. Unfortunately most of those that fail out early are the non-English speakers. The lectures are 2 hours of rapid furious talking-that is so fast you can't write it all down. Tough school.

Does anyone know of any site or information that is culturally specific for Hispanic families regarding maternity: Ante/Intra/Post partum...most specifically Puerto Rican. -Thanks- Helpless in NY

What should I be aware of when medicating a Patient with Beta Blocker therapy(Metoprolol/Lopresor)? I was asked by a Professor and I am stumped.

The Master Cardiology is defintiely not a Double bell. It uses a compressable single bell which reproduces the smaller bell when it is pressed with a little more force against the skin. The double bell I have found in emergency situations presents a problem by always being in the wrong position and at time you have only one hand to use.

I like yourself am changing careers. I would like to go to medical school but I have 3 kids. All my plans involve feeding the family. I am doing a 2 year RN program and then looking to do a BSN. There should be a transition program into MD for RNs. They have spent 4 years with a focus on diseases, treatments, patient care and assessment. Meanwhile I see young, mostly foreign students with limited English and people skills doing residencies. There definitely is room for some BSN to MD programs.

I cannot speak for the rest of the country but in the NY area. The clinical component of the programs are identical 4 semesters of hands on learning, classroom theory, lectures. In the 4-year schools the firstl 4 semesters of nursing are filled with non-nursing related pre-requisites, ie; English, Sociology, Statistics. I have compared course content with other local students and the nursing component seems to be identical.

Another "Nancy" encounter: I came in early-as usual-the other night and "Nancy clone" couldn't wait to jump off her more than ample rump and excitedly tell me "Don't even bother putting your stuff down you got floated to ICU..HAHA!" I asked if she was coming in in the moring She said "Yes why?" I returned 'well then that is a plus side to this story...I won't have to see you!' The entire desk burst out laughing. The ICU coordinator came in for the AM and offered me a position. FIGHT THE NANCIES!!!

I am sure the hospital is aware that collecting on this "buy-out" successfully in court would be next to impossible, even if the employee signed the contract.

My physical exam consisted of a Drug/Urine screen and a set of vitals. So don't let them put a cuff below your knee and no pot smoking with your bad foot. LOL-good luck

Why don't you contact some places before moving and see what they say. And I mean call the unit on which you want to work, a "civilian" secretary can yes you to death but she doesn't hire. I think 62 is a median age aspecially with all that experience..they would probably beat your door down to get you.
