Yes, I am considering it. I'm a very experineced LPN with ED experience so that should help, we'll see.

"will continue to monitor"
I also don't use "No change" because if something happens how can you prove no changes unless you assess the pt . For examople, can there be no changes to lung sounds without listening again to say no change.
-
"will continue to monitor"
I don't use "will monitor" because then you must prove with more documentation you did.

I hate it when the male nurses on my unit...
I've never heard of such a thing. Yes, if a pt request it, but it's rarely happens.
-
Fear of physicians and taking telephone orders
As wrong as it is, it's the true.

Fear of physicians and taking telephone orders
Comes with time and experience. Sound like you know what your doing makes it easier. Side note: I believe male nurses are recieved different then females. This includes getting orders.

What would ja do?
I think it could go two ways: 1)start it and then have the day nurse follow up with md. 2-3 hours not going to send the pt over the edge. 2) call MD at 7am to clarify. If your not going to carry the order out, you have to have a justifiable reason. I'd probbaly would have started it myself especially if the pt is NPO and hasn't drank or going to eat breakfast.

New nurse here...slapped today by patient.
Technically it's assualt, yes. My point is that the hospital probably would never do anything if the pt is elderly verses young. Right or wrong certain situations carry various reactions with people. For example, if a 80 year lady is hitting on me, which is assualt, and I clocked her one time in self defense I'd be fried chicken. if the pt was a 20 yr old man attcking me and I clocked him one good time it wouldn't carry the same impact verses grandma getting clocked. Hopsitals do indeed put image high on the list even if it means not backing staff members at times. So filing charges against an old lady who slapped a nurse would not be supported by the hospital, because it would hurt the hospitals image.

When they are that old not much is going to happen. you'd have to get clocked by a young man or something before someone would do something.
