Please help! Was this a hippa violation?
We do not have one, we are a small privately owned facility. What are the patient's options? Can we be sued for it? It was a nurse who sent the papers, can her license be in jeopardy? The patient was fired because she told them a different diagnosis than what was on the chart. The diagnosis she told them was one of the differential diagnoses the doctor counseled her about due to her symptoms.

Please help! Was this a hippa violation?
A patient's employer called today stating that the patient was there with her, to start working after being given a few days off by our doctor. They want to make sure she is not contagious to their own patients. The record was faxed to the employer and now we found out the patient has been fired. The doctor told us it is not a hippa violation because of the fact that the patient had given OUR information to the employer and that is an implied consent that they can discuss her condition with them. Is it true?

Who is responsible to find shift coverage?
Thank you guys, i see your point of it not being a sick call and was known two days before the date. That was a point i wasn't able to bring up with her. To her, I'm the manager and should be responsible regardless of the reason. Then what's the point of fixing the schedule to benefit everyone? I might as well do the schedule however i want if i end up picking up their shifts anyway. Of course thats not something i want to happen. Employees should still have some semblance of responsibility to their shifts unless they have a valid excuse such as being sick or had a real emergency.

I was fired by an MA at orthopedic clinic
When i was first hired at my current job, there was an MA who had been there years before me. She made my life miserable, bad mouthing me to patients and other staff about me. She was mad that I got paid higher than her even though i had no experience because i had a BSN. After two years she was finally fired because she would be rude to patients, sleep on the job, come to work hungover etc. I say you are better off not working there. You will later find a job that has a not so stressful environment. One that does not endanger your nursing license.

Who is responsible to find shift coverage?
Do you work as staff nurse as well like i do? If I was a traditional manager or supervisor who oversees the schedule, then of course thst would fall under my responsibility. Unfortunately, we do not have that setting.

Who is responsible to find shift coverage?
Unfortunately there's hardly any distinction with an MA and RN because we all do everything from front to the back. And this MA was hired only a couple months after i was hired 4 years ago. We are close so my husband thinks maybe she is taking advantage of that. The only person I report to is one of the 6 doctors who is the actual owner. I'm planning to draft a little policy regarding this and other matters and have it looked over/approved by that doctor. This will make everything clear and will also benefit the two new employees we are about to hire. Honestly, it looks like they have no plans of finding a new manager. I am grateful that I was given a chance to do this but I was not prepared nor do I want to give up my other job because of this.

Who is responsible to find shift coverage?
I work in a small, privately owned urgent care. There are two nursing staff (use term loosely because I'm fhe only nurse left and the rest are MAs) and one doctor working each day. Our nurse manager was fired a few months ago and being the only nurse left, i was handed the position as interim. Basically the extra responsibilities would be to order supplies, make the schedule etc in addition to working my own 3-12hour shifts per week. a situation came up where one of the MAs asked someone to cover her and that person said yes, only to take it back after a few days. I got into an argument with the first MA because i told her it is her responsibility to find someone to cover her shift since the other one cannot, otherwise she needs to come in. She cannot find anyone else. For her, since i am "manager" i should be responsible. Obviously, we do not have a traditional manager-employee setting because I work as staff as well. The month before i make the schedule, i let them put in their requested days off with the agreement that once it is scheduled, it is their responsibility to work their shifts. Am i in the wrong here? The reason i cannot work is i am scheduled at my other job already (which i've had for 2 years before this happened). She cannot work because she has a test coming up (going to school for nursing).and she is not prepared. oh, and I'm working 9 days in that two weeks already to cover her other shifts.

Cost Cutting/Budgeting --- the patients suffer the most
Hate to say this but that is exactly what some of my duties are. I work in a private urgent care and they don't hire cleaning people because they expect us to literally fill that bucket, mop the floor and scrub the toilet. Also do patient registration that includes insurance verification and patient call backs the next day. Granted, we don't get that many patients, average of 20 in 11-hour shift (2 nurses and one doc all day). And our electronic charting system is easy and fast. Oh we also do xrays and lab tests ourselves.

RN filling in for receptionist, workflow.....anyone else?
Oh, just to add. We get paid if we stay over. We never have to stay longer to do receptionist stuff though. It's usually because patients come in late and we have to finish taking care of them. How is it that you don't get paid like that? I guess its different because we are a private placE?

RN filling in for receptionist, workflow.....anyone else?
I work at an urgent care center where we have 1 doctor each day, open 12 hours a day. We see average of maybe 20 or so patients a day which could go up to 40 depending on the season. So it's the doctor, me (RN), MA and billing person (up to 3PM). Me and MA (or any of other nurses and MA) would do everything from front to back. Basically, act as receptionist, nurse, and even do the charges for the patient's visit. The billing person would send the claims and call to collect balances. We all answer the phone whoever can get to it. Surprisingly, I love that I am able to do everything. The registration takes about 5 minutes. I swear we have the easiest system! That includes verifying insurance online. The MAs know how to give shots, do IVs and we even xray. Granted the duty lines are blurry but as a small, private place, it works well for us. We can get the patient in and out in 15 minutes. Sometimes instead of an MA, I work with an ER tech or paramedic and they prefer to do the back part (we all need to learn everything) because they type slow if they do registration so I just sit, look pretty and register patients. I help them of course if it gets busy but for them I'm doing them a favor for doing the registration part that they don't like so they let me relax (never at the extent of making patients wait of course). So I can't say I do not enjoy getting an RN salary for being a "receptionist" sometimes. It's kinda messed up if you think about it properly, right? I work at another urgent care where we see 40 pts in 12 hours and I only do the nursing duties (I am exhausted at the end of the shift!) so doing receptionist stuff is a good break from that.

Is it possible for a RN to work five 12 hr days?
Yes, I've been doing that for about a year now. I work 3 12hrs as FT in an urgent care and another 1-2 12hrs PRN at another urgent care. I am 27, married but not kids yet so I feel like this is the perfect time to be doing this. The perks with my FT job is it is only literally 5minutes away from home and it is not that busy. For my PRN job, it is 30mins away but I get paid $10 more an hour. You can do that especially if you do the other one PRN because you can choose to not come in for one week if u need to. Plus the enviroment is different such as your computer systems and such so you won't get as bored as compared to doing 5 days in the same job, I would think. Sometimes I get scheduled at both places up to 9 days in a row if I want to take a little trip and want to be off for some days in a row too. I got used to the 60-hour work week. Just make sure you eat right and get your rest too. The money is not bad either. :-)

Bully?! Help,I need good comebacks!
Thank you for everyone's advice. I can't wait to work with her again and see if she still does it. If she asks me one more time, like, "Can you put her in a gown?", I plan to just say, "Would you?" How does that sound? One poster is right, I'm not one who really makes smart and quick remarks, so I want to do just a plain, direct one. Thanks for that advice about googling assertiveness. I'm going to try that too.

Bully?! Help,I need good comebacks!
As far as I have seen in this urgent care, and where I work at full time, patients are not changed into gowns unless they need an x-ray. Chest x-rays are not as easy to predict as, say, an ankle or hand injury. I've come across patients who I expect needs a chest x-ray and the doctor doesn't order one. It would be pointless to change everyone with a cough into a gown. But thank you, for giving me an insight to a different way of thinking. It helps to have objective people. To be sure, I also asked the manager if the patient usually changes in the room or the x-ray room, and she said the x-ray room. The x-ray room is a the very end of the hall and if you walk in a gown, you will be in plain view of everyone. I didn't mention though that Ms RT is ordering me to do it each and every time.

Bully?! Help,I need good comebacks!
I suppose I failed to mention the snide comments she does especially around other people, which to me is a form of bullying. Mostly personal ones, example, our ID has a BIG RN on it. And one time she goes, "Wow, they really put a big sign on your ID so people know you are the nurse. I guess that helps because you look like you're 12. " If it was meant to be funny, none of our other coworkers were laughing. It was a big awkward moment. And that's not all. I guess this is an example of what I meant when I asked for comebacks. I try not to let it get to me, the big issue I have is the delay in patient care because she wants to wait for me to do part of her job. She's the only tech who orders me to do that. And I'm the only nurse she does it to.

Bully?! Help,I need good comebacks!
I just started working PRN at an urgent care. Im an RN and there is usually only one nurse on duty (me) , a rad tech, lab, doc and front desk. We are usually busy..Lately, I noticed one of the rad techs keep telling me to put patients in a gown so she can do the chest x-ray. Now, Im new so I'm not sure of how they really do things there, but the other rad tech never asks me that, she gets it herself. Of course I don't mind doing that to help her when she's busy but the thing is, I'm the one always busy since I see ALL patients and she only sees people who need x-ray. I'm mostly in patients' room so I'm not usually the first person the doctor tells if he needs an x-ray (especially chest, since it's not as predictable as extremity injuries) and so it creates a delay because she waits for me to put patients in a gown. Really?! I asked another nurse and he said that is part of their job. We can help, sure, but they shouldn't be waiting for us to go get the gowns. Apparently, he never gets asked to do that, just me. So now I'm thinking, I might be getting bullied for being new. I really need good comebacks the next time she orders me to do something she should be doing. I'm young and kinda shy and is such an easy target for these, I admit. Please help! I love this job so far and I don't want to lose it.

undeserved promotion?
Just giving you an update that I got it!!The MA just kept quiet when it was announced and she was the only one who did not congratulate me. Oh, and the billing person kept quiet too. She's the one who talked to the doc and said she thinks she should be the one promoted to my position, but he said, you work in billing, you are not a nurse. :smackingf sometimes, i cannot believe some people's logic.

Fed up with a tech. Long- sorry.
I LOL'ed at this! You should've said ..since we all know they are higher than some "lowly" NP. LOL!
-
Fed up with a tech. Long- sorry.
Wow! This is your attitude as a YOUNG NP? I wonder how much worse it will be after a few years. Need to learn some humility. If you realize someone higher than you could possibly be making a mistake, would you stand by and let it happen just because you are of lower rank? There's nothing wrong with people asking questions, it's scarier if nobody would ask questions because they are intimidated of rank. Even presidents/kings have advisers.

I'm glad there are still good leaders out there!

Things you'd LOVE to tell coworkers...and get away with it!
Umm...RNs make more than Medical Assistants no matter how many years you've been there, sweetie. (not a bash on MAs, just a particular one who's been complaining about this)
