  1. I used a wrist cuff for general use in LTC, but like NamasteNurse said if its close to a not ok bp, I always got a manual reading for legit use.
  2. Could they schedule a med aide to come in at 0500? I know its just pushing the issue off on someone else, but that was the solution we used and it worked well. We could count the med cart at 0500 and the med aide didnt have to rush the residents like I had to. Plus the residents are already familiar with her. And day shift recieves report on time. Just an idea...
  3. the lable provided states: iv (adult with chf) loading dose-4.7mg/kg not mg/kg/hr it calculates out to 319.6 mg (safe loading dose) per hour the order doesn't say anything about it being per hour. the looading dose was ordered as 300mg. you figured 316. that is why the order is safe to give. do you see it. i know i always got hung up on labeling, looks like that is what is going on here.

  5. Coming from the LTC standpoint, we always sent a CNA with the pt. to dialysis for ADLs. That way someone is there to clean them up, help with blankets, repositioning, etc. I thought that was common practice.
  6. We had this issue in a LTC facility I worked at. We restructured report in a book that had updates of conditions that had arose from the last month. I think a fresh set of eyes for the nurses and patients is a good thing. We would rotate halls every couple of weeks. This prevented burnout from the nurses and gave the residents something to look forward to. Some of our nurses became complacent and missed some crutial things that shouln't have been missed. I really enjoyed that system.
  7. I drive 2 hours to work and am there 12 hours Friday, Saturday, Sunday nights . I stay with a friend who is gone on the weekends. On day shifts I drive back and forth, but it just about kills me. I am married with kids. It works perfect for my family. I am able to have the weekdays off to help my kids with homework and after school activities. We have a couple other nurses that drive an hour and a half or better, but by the 3rd day they are zombies. I don't think I could drive everyday... HTH
  8. We have called paramedics in our ltc faciliy to start IVs before. I never thought twice about it...
