How difficult is FNP school?
I think you already know the answer to the question. The evidence is every single NP, I have met who came from the NP community has said the CRNA school is much harder than NP school. I do not think there should even be a debate about this. Why is it so difficult to admit one is simply more difficult than the other? It is just a fact of life. I think egos are being bruised.

The SRNA thread, as is the CRNA thread, is very moderated. They are only as insulting as the moderators will allow. Why don't you ask a moderator to tone down the replies over there? I would be willing to bet the post I quoted above would not be tolerated in a moderated forum.

Do you need hospital experience to function as a NP?
It does not appear that any hospital experience is required. There are direct entry NP programs. And while TECHNICALLY hospital experience, albeit, not patient care experience, I have seen many an operating room nurse go to NP school with no real patient care experience. While it might be a good idea.. it is by no means an iron clad requirement. So it would appear the real answer is.. no you do not.

Organ Harvesting Anesthesia
Non brain dead organ donors are those who do NOT meet brain death criteria, BUT are going to be "terminally weaned" from the vent. In other words support will be withdrawn. The patient is taken to the OR, the wean is done and once the heart stops, the organs are harvested. Yes it is being done and is quite controversial to say the least.

I must say, I have to agree with him or her about the lack of critical thinking skills. I lay the blame on the maternalistic manner in which most nurses are taught... and then managed once they graduate. I do not think malpractice is a cause, as suing a nurses is pretty rare. Many nurses blindly follow protocols without questioning anything. If you are given a stupid protocol to enforce, you need to say... wow this is pretty stupid, I think I will find employment in an environment that does not take me for an imbecile. Let me give you an example of a lack of critical thinking skills. I do neurosurgical anesthesia in a huge medical center. The "Desk people", these are nurses who do not like patient care and sit at the desk all day with headsets on like air traffic controllers, have been given the task of insuring EVERY SINGLE PATIENT has a surgical cap on before they are taken to the room. Did I mention this is neurosurgery? they get back to the room , the cap is removed and the head is shaved IN THE ROOM. That, my dear friends is a serious lack of critical thinking.

What gauge IV to use?
That's all good in the OR, but outside of the OR, there is very limited use of local anesth. for IV insertion by staff RN's. It is an ongoing issue of whether the RN can administer a parenteral (IE: non topical) anesthetic prior to IV start. There is no common consensus between state boards of nursing. I would think a standing order in place from the medical director of your ER or the ER physician on duty would be more than adequate. Do you not have chest pain protocols that involve at least an aspirin chewed on presentation? This is along those lines. Topical carbonanted lidocaine 0.5% or 1% is pretty safe for just about anyone. And local anesthetic will actually improve your chances of getting the IV.

Trivial? I would submit what they are doing is almost worse than physical abuse. To take a group of people, many of which are of a diminished mental capacity, and preach voodoo, complete with chants , hang prayers in their rooms insinuating they are sinners , tell them they are going to hell if they do not accept a 2000 year old dead rabbi as their personal savior.... and THEN hit them up for CASH? Mountains out of molehills? I think the OP is actually underestimating how dangerous this is. I can assure you, if I had personal knowledge of this, I would not have asked on a discussion forum what to do, It would already have been done. We are wayyy too tolerant of religions bilking people in this country. It is time to fight theses charlatans.

What gauge IV to use?
With local anesthetic... 1 or 2 % lidocaine with bicarb intra-dermal prior to insertion.... a 14 guage feels exactly like a 23 guage in the hands of a veteran practitioner.

Alternative Airway Knowledge
Cutting the pilot tube as a student will get you a very poor eval and kicked out of the OR to go look up articles. Cutting the pilot tube is darned near malpractice. The pilot tube on endotracheal tubes made since about 1974 are NOT elastic and self deflating. if you cut the tube and remove the tube... guess what? You are deflating it against the cords.. Medline some articles about vocal cord injuries and ET tube removal.

Allways bigger is better. Here are the flow rates for IVs 22ga X 1 inch =35ml/hr 20ga X 1.16 inch = 60 ml/hr 18ga X 1.16 inch = 105 ml/hr 16ga X 1.16 inch = 220 ml/hr 14ga X 1.75 inch = 330 ml /hr Notice how the flow rates increase almost exponentially? It actually increases by the power of 4. look up the Hagen–Poiseuille equation or law. Avoid the AC unless you're placing a RIC line those are 7 or 7.5 french. No one ever said " Darn I wish I had not put in a big IV." Nothing wrong with the dorsum of the hand.... it allows you to move up the arm if the IV infiltrates.

Unless the nursing home is funded by a christian chruch... and EVERYONE has the right to REFUSE to listen to preaching nonsense.... what they are doing is wrong. It is a captive audience and, from your description, without the capacity to make an informed decision. Like most religions.. they are preying ( no pun intended ) on the weak minded. I think I would drop a dime to the state or federal entity that certifies LTC facilities. These people must be STOPPED. this is bordering on criminal in my opinion.
