Co-worker making numerous mistakes
Thank you, up to this point everything reported to the DON has been verbal but that will change now. We are not allowed to do an incident report or a med error report on another employee, only the managers can do that!

Co-worker making numerous mistakes
I work in a ALF. We have a prn LPN that has been a nurse for over 25 years although she has not always worked as a nurse. She worked for our facility when we first opened then left to go some where else and has now returned as prn but she works frequently. My questions is, since she has been back she has made numerous errors that have been witnessed by other nurses. We report them to the DON and she either laughs them off or says she will talk to her but acts like she doesn't believe this person could be making these errors. When she does talk to her she pretty much lets her know who reported the error and then it difficult working with her I know the DON is not handling this professionally but just how many errors can be made before you go above the DON? I am not talking small errors either, in 1 morning she did a accucheck on a person who is not diabetic because she got the people mixed up, brought me a syringe to verify an insulin dose and had it in a tb syringe, gave a person a ativan at 8am when they get it only at 12n and 9pm.

Incident reports...how long does it take to complete one?
We have paper, if we don't send them out we have a 1 pg incident report, 2 page fall followup such as what could have been done to prevent, paper to notify the Dr is not serious and charting along with notifying the family. If they go out there is another 1 page form for the state along with all the previous.

Assisted Living
I work in a ALF with 24 hour nursing. We have a LPN that works 7a to 12n and one that works 7a to 7p. Then we have a LPN that works 7p to 10p and one for 7p to 7a. We have a CNA 7a to 2p and one 7a to 3p. The 3p to 11p and one 4p to 9p. Then we have just 1 CNA 11p to 7a with the LPN. We have DON but no ADON.

The meds that we give our residents come in bubble paks like in the nursing home. We are such a brand new facility that at this moment we only have a DON/Nurse Manager, 3 FT LPN's and 2 PT LPN's and about 6-7 FT and PT CNA's. We have assisted living in the back of the building and independent living in the front. We have 14 AL residents right now and about 25 independent. Right now we have 1 LPN and 1 CNA per shift except 11-7 which is one LPN. Until we get a few more residents, the LPN and CNA are responsible for heating and serving the meals, laundry, cleaning apratments and the building, activities and the LPN has med passes. It is a nice variety but I will be glad when we are able to bring on housekeeping and more dietary help!

Assisted Living Center
We have 24 hour nursing on duty, not on call but actually in the building. I don't know how common this is but in our town none of the other alf have it. We don't have any med techs, only LPN's can give the medications. Thanks for your reply!

I am currently working at a alf that has been open about 1 month. Brand new beautiful facility! My question is what kind of activities do you do with the residents? What meds are in lock up? Everything seems so murky compared to waht I was used to in nursing home setting. Any ideas would be greatly appreciated. We do have medication assistants, we have an LPN and a CNA on the shift right now, we have about 16 independant and 6 assisted in the facility. What jobs are you responsible for? Do you have dietary and housekeeping on 2nd shift? All kinds of questions!! Thanks everyone!

Assisted Living
Sounds great thanks! We are not having any specialized units. We have 32 bed assisted living, and I think a 38 bed independent living. This building was just built and won't open for a few weeks but it beautiful inside. A person wouldn't have to leave! They offer pretty much everything the outside world has including Chapel, Beauty/Barber shop, exercise room, billards room, pub, soda shop, 4 different dining rooms, theater, etc. I am very excited to get started with residents, thanks again for the input!

# of residents in LTC
I have approx 48 residents on 3-11 shift. I have 4 cna's and me and I do all treatments, med passes, dr orders etc. This is to many to give any kind of care to let alone quality care!

What's It All About, ALFie?--Help and Support for Assisted Living Nurses
I have been a LPN for 18 years and I have a interview tomorrow morning at a new retirement community opening in my town in December. They have assisted living along with other styles. I am currently working in ltc and really want out! Another nurse where i currently work had an interview monday and she said they didn't discuss salary as they said they were not sure what the pay would be, they also told her that they expected the LPN's and CNA's to clean the apartments top to bottom once a month. I don't have a problem with that but she said it was not for her. My question is, right now I make 20.35/hr, anyone have any idea what ALF's pay?

My first resident passed away last night
Just talking to people who understand. I lost my first resident last night. I have been a nurse for 18 years but only in ltc for 3 1/2 months. This was a special one, :heartbeatI admitted him and have been his nurse ever since. He came in up walking, talking, joking going to dialysis 3 times a week. I knew he didn't have alot of time, he was end stage renal and multiple myeloma but I didn't think he would go so fast. I had gotten close to his wife also. I cried :cry:off and on at work last night throughout my shift and it seemed like everyone else went on like nothing happened:crying2:
