Woman's Hospital of Texas interview
Wow! Congrats on getting the job, I too would love some guidance on what type of questions were asked, what they were looking for etc., Did you just email the recruiter your resume and call her repeatedly to ask if she had a job opening? curious as to how that works when they don't have openings posted....

I'm trying to round up the links to the Houston areas summer externships for those in their last year of nursing school, so far I have only found the ones for St. Lukes and M.D. Anderson. --anyone know of others?

IV Piggyback
?? you wouldn't give 1mg/0.5ml of dilaudid? we regularly give 2mg/1ml ivp dilaudid, were you referring to something else? as a side note, another thing i've learned with giving this is to pull up the dilaudid into a syringe containing 8ml or so of ns, flush with 5ns before and after. the dilution acts as another safety measure so you don't get this drug in a bolus shot all at once.

Interventions for Acute pain Dx
Patient has had bilateral mastectomy, was discharged home and came back 2days later when one of the drainage tubes came out by accident. In pain with swelling on R side which MDs are assessing for necrotized tissue. I have put "Acute pain R/T invasive surgery AEB pain at an 8 on a 10 point scale" as the primary nursing diagnosis but I am kind of stumped on what interventions to use other than administration of prescribed pain meds. Help!

Care Plan feedback
working on my first care plan and thought I'd throw it out there to get any feedback on improvements needed, items lacking, clarification etc., etc., Pt. is 89yr old female admitted for dehydration, Hx of cervical cancer (receiving radiation therapy), HTN, Neutropenic. Prioritized Nursing Diagnosis: 1. Deficient fluid volume related to active fluid loss and inadequate fluid intake, as evidenced by persistent diarrhea and inability to drink sufficient liquid volume secondary to fatigue. 2.Risk for infection related to decreased white blood cell count secondary to radiation therapy as evidenced by laboratory data of wbc=1.4. 3. Risk for impaired skin integrity related to persistent diarrhea as evidenced by redness and moisture in perineal area. still working on getting the goals and interventions for the #1 diagnosis. Any thoughts? tia

PRBC's for Hypovolemic shock?
That totally makes sense now, Thanks!! In my head I was thinking "hypovolemia" which would be a loss of fluids right? and you would want fluid along with the blood cells, but as you explained it "hypovolemic shock" is different in that it is a loss of blood volume not fluid. correct? Thanks Daytonite! I appreciate it!

PRBC's for Hypovolemic shock?
Can anyone explain to me the rationale behind giving Packed Red blood cells to a patient with Hypovolemic shock? this is a question from our patho/pharm class and I am having a hard time understanding why, in my head I am thinking you would want to give whole blood to replace fluids, but maybe there is something I am missing.... she also asked why give albumin to patients in shock? I am thinking because albumin will draw fluid back into the cells, but I guess it would depend on what kind of shock right? Thanks in advance for any clarification!

med calculation problems, mg/kg/day

med calculation problems, mg/kg/day
i need some help in figuring out calculation problems that involve weight and dosage, i use dimensional analysis for all other calculations and have no problem, but don't quite get the g/kg/day ones! example 1: pt weighs 220 lbs. administer digoxin .125 /kg/day. administer___mg(s). example 2: administer tobramycin 1 mg/kg/day bid. available is tobramycin 10mg/ml. the patient weighs 110 lbs. administer ____ml(s). example 3: administer ampicillin 0.2 g/kg/day q 6 hours in even doses. available is ampicillin powder when reconstituted it will deliver 1 gm per ml. the patient weighs 60 kg. administer ___ ml(s) at 0800. :) if anyone would be so kind as to lay these out in a da format i would be very grateful so i can learn how to work with these types of questions! tia! p.s. here is another problem that is giving me grief, though not related to weight calculations. administer demerol 75 mg & nembutal 100 mg q 4 hrs prn pain. demerol 100 mg/ml and nembutal 200 mg/2ml is available. administer _____ml(s). --do you think this question is asking for total mls of both meds? or individual doses? or what? thanks guys!
