  1. Good evening, I am trying to get a little information. I work in an institution where high risk needs to be double checked and signed by two RNs independently. The process is to verify patient, drug, and dose. The nurses are running around extremely short and with severely high acuity patients and there are still med errors even with this process. I am trying to understand or get input for what is done at other institutions?? How are high risk or even standard med errors eliminated??? We are not a facility that is computerized yet we are still using paper charts...help please
  2. I have I attended a conference in New Jersey and I believe it was very beneficial.
  3. I don't think so, because in icu you will also learn about different types of drugs.
  4. Don't be so hard on yourself since you are new to the unit it takes time to get your own system down.

Hi...I was recently at a rapid response..where a patient was have runs of accelerated svt. The doctor and a ton of other medical personel were at the bedside. After carotid massage, the doc decided to start the patient on a amiodarone drip after the 150mg bolus was given. The 150mg vial was already available on the code cart. The issue was instead of going to pharmacy for the 10 minute bolus that is still 150mg. How come we just couldn't give the 150mg IVP? And then hang the drip?
