Giving labetalol with low heart rate
It never hurts to question, because it's impossible to take it back once you've given it! I would have held it and talked to the pharmacist to work through mechanism of action with him, given the concurrent hypertension and the bradycardia. (We have tele-pharmacy, which is a great option overnight.)

Nurse Consulting (Self-Employed)
I would love to hear from sole member LLC/small business/self-employed nurses. What are you doing? Why? What do you hate? What do you wish you were doing? Why? I really love what I do every day, but when I look at how much I have to set aside for taxes, travel and business operating expenses, and how much we pay for family health insurance ($1,850/mo) now that I am not carrying family insurance with a regular job, I want to cry. (Can you tell I've been wallowing in tax documentation for the last two weeks?) Anyone who would like to commiserate, give encouragement or give me a kick in the pants, please weigh in. Thanks!

Why I guard the referral telephone like a dog...

How do you decompress/debrief?
Thank you all for the great ideas! What I am taking away most is that the divison between my work self and home self may take conscious effort. Great...more work:) I have a 1.5 hour commute and usually that is enough, had the Abba cranked on the way home and the sunroof open...but I think maybe additional routines are in order! Love AN....thanks friends.

How do you decompress/debrief?
I am a new ER nurse (3 months) and new nurse (2 years)...I am just wondering how to stop thinking about work when I am not there? I find myself ruminating over my mistakes and over what I will do differently, dreaming about work, not enjoying my days off, and I am not mentally or emotionally here for my family. Any suggestions would be helpful, thanks!

fired... where can a bad nurse go to find a job?
What about something like Protime nurse in a Coumadin clinic? It is very repetitive, and when you get good at it you likely will have a feeling of satisfaction in your role. You build lasting patient relationships because some of them have to come every week. There is a patient teaching component. I found it difficult to get the hang of for the first month or so, but then as routine as phlebotomy once the adjustments became second nature. Just a thought!

I'm New To ER...Share some tips please?
Congrats!!! I am anxious to hear what responses you get, as I am on day 2 of orientation for my new ER position. I wish I had YOUR knowledge! (I guess the matter is always greyer in the other nurse's brain!)

Full face mask bipap
I have never seen any other kind...the full mask was all my floor (resp. emphasis) ever used. I had two pts who only needed them overnights that would ask for them at 8p on the dot; they seemed to really look forward to the decreased work of breathing. Two other pts whose families used them to buy time while deciding what to do (multiple comorbidities); eventually decided to d/c and pts eventually passed. Just have to make sure they are properly seated and don't cause abraisions to the face (though I guess that is true for all masks), watch for n/v, and take it off to offer po water. They are difficult for the pt to remove themselves if pt is not A&O x3.

How aggressive is your follow-up?
I was just browsing when I saw my thread and thought I'd post an update...I GOT THE JOB!!! They called 6 weeks after my original interview, out of the blue. May 19 interview...August 1 start date. Patience is the name of the game.

Do you shower right after your shift?
I would not have the energy.
-
Love it!
You give me hope I will love it. Thank you! I start my ER adventure Aug. 1! Nervous, excited, and can't wait!
-
What's the longest time that passed before hearing back that you got the job?
6 weeks after a single interview, they called and offered me the job. I had stated that I needed to give one month notice at my current position, and they called with three days to spare, given their anticipated start date.

Is it true? BS + RN does not = BSN?
Exactly! Thank you. Usually the smaller schools have a nursing department that is a subdivision of the school of science. Larger schools have a school of nursing, that is an entity unto itself. Both are bachelors qualified nurses, and there is no distinction between the two. RN, BS (bachelor of science) = BSN (bachelor of science in nursing).

Preceptor from hell- no more killing with kindness
I had almost this exact same experience at a clinic a year ago. It was not my primary preceptor but I was to spend a few days with her and they gave her no notice. I ignored her behavior that first day and let her have her space, working more closely with another nurse in the department. The next day she apologized profusely. It turned out it had nothing to do with me and everything to do with her being a type A control freak over her workspace. She ended up being one of my favorite nurses...what was a detriment when it came to precepting was an asset with regard to her ownership of her role and attention to detail.
