Please refer to Federal Tag 221. This has to do with Physical Restraints. By definition, a physical restraint is ANYTHING that is fastened to or around the Resident that prevents the Resident from rising or having NORMAL access to their body. A low chair is not a restraint Unless, the Resident cannot rise every single time and on demand. Hope this helps.

CSM guidelines for Pressure Ulcers
It really isn't that you have to prevent the sore.. If a resident is at risk for a pressure ulcer, the facility must have a plan of care in place TO TRY and prevent the sore from occurring. This is where a great Care plan coor. would come in handy. If the facility has put interventions into place and the resident still develops the wound despite the facilities best efforts, then the sore must be tracked, the MD must be notified of healing/non healing, treatments that aren't working must be changed, etc. Hope this helps. I did read the big packet..haha If the facility fails to to do any of this, they could potentially be looking at a harm level citation.

undefined hi nurse mid.. i was a don for 6 years. it sounds like you are putting out small fires and can't yet see what is causing the blaze. first things first..hold a mandatory staff meeting and let the staff know what is expected, what will and will not be tolerated. be very clear in your expectations. next, evaluate the results. you will know rather quickly who wants to be on the team and who doesn't. what was the last don like? perhaps she/he was very lax and didn't expect very much. it is also very important for the staff to see that you are buying into your own program. i have seen many don's with the attitude of "do as i say, not as i do". being an effective don takes a lot of time and hard work. to be an effective (and respected) don, you will spend very little time in your office and alot of time on the floor. how large is your facility? there are many things to think about, priortize what is most important and don't try to fix everything at once. i wish you all the best.

Chad, You are your resident's best advocate; if you feel this strongly that there is wrong-doing, you should be telling someone other than the people in this group. The number for your area Ombudsman should be posted in the facility- give them a call and be very factual.

Yes, we do come in on off hours. We are mandated to do so.
