I worked in dialysis 3 years before getting into home health. I really enjoyed dialysis at the time because it was mostly routine once you learned it. It did become redundant and the income was just mediocre. When a home health job became available I decided to jump on thr opportunity. I'm very glad I did. I love the flexibility of the job. I also love the one on one time with my patients and being independent in my job. The charting is the only con with home health. It is a lot to learn but not too bad once you get it down. Things change a lot in home health too. Overall I wouldn't go back to dialysis or hospital after being in home health for 4 years. They say you will love home health or hate it.

Glad to hear about the kci prevena, this particular pt did not receive that particular brand of vac. Dr did not specify which brand pt needed or type of dressing in particular. I will mention this to surgeon. Thanks so much for the info.

I have been a home health nurse for 4 years now, and today I had a pt come home from hospital with a closed surgical incision that was cellulitis and abscess, closed with sutures, very minimal space open, no drainage. I have only seen this ordered one other time, and I will tell you it was a disaster, pt ended up back in the hospital for wound infection d/t wound had enlarged. Called the surgeon, he states do as ordered place device over si because this is for lymphatic drainage. The dme company stated they were under the impression that the wound was open, not closed, and that this is not a normal practice for negative pressure devices due to the fact that it does indeed cause a new/worse wound in most cases. Just curious if any of you all have heard of md's ordering this treatment for closed si's related to cellulitis and abscess. Very frustrating for me and the pt.

Hi, I am a new grad as well, in May 08. I started my first job in Dialysis on May 23rd. I love it! I trained as a tech first, then my nurses training began. I have been off of orientation for a week now and I love it, but it is still very scary because orientation is never long enough to learn everything you need to learn in Dialysis. But I work with some really great people who are helping me out everyday. I absolutely love my job, and most of the nurses I work with started in Dialysis as well, so I dont think its a problem at all! Good luck!

The first time through, I just introduced myself and stated I would be back soon. That way, I could make sure that everyone was where they're supposed to be (not on the floor) and nobody was in acute distress (my first priority on everyone). Then, 2nd rounds: I'd go back through and do my assessments. 3rd rounds, med pass and taking care of 'creature comforts'. You can't 'take the time' with x pt when you don't know anything yet about 'y patient'. I never stopped until my 'first rounds' were complete. Now the other job, which I had the offer today after my interview is at a dialysis center. It would be 4 days a week, then 3 days the next week. I would have quite a few patients to take care of during my shift. I would be getting paid 18.50 to start and then 19.00 after ninety days. I would not have to work sundays, christmas, thanksgiving, or new years. I just cannot decide which one to go with right now, I am weighing out the pros and cons. Someone please give me some advice! I am really stressed out!!! The dialysis place is holding my job for me until monday. I need to call her monday and let her know something.

I am in need of a favor to ask a question. It is probably going to sound like the dumbest question you have ever heard of, but here goes. I am in OB nursing, and I am doing my care plans on my patient who had a C-section. I am using Acute Pain, Risk for Infection and Knowledge Deficient. The Risk for infection is throwing me off because I cant figure out my As manifested by or evidenced by's. I am doing Risk for infection related to abdominal incision AMB..... I cant figure that part out, we didnt use Risk for diagnosis in fundamentals and so this is throwing me off, any advise suggestions would be much appreciated. I have to finish this by tomorrow night. Thanks so much!

80% Is passing here, but our school was just recognized yesterday for the only school in the state to ever have 100% pass rates on all the allied health programs boards, certifications etc. So I am not going to complain about it!

I have to come up with three Diagnosis for my patient I had in clinicals last week and do careplans for each. I have a few ideas but I am having a hard time coming up with my related to's ( I always have problems with this). My patient was having severe chest pain and later on that day the doctors told him that he had an MI the night before(they think it was the night before). My three diagnosis I was going to use were, Acute Pain, Inneffective Tissue Perfusion, and Activity Intolerance. My teacher suggested using Shortness of Breath as a related to for the pain, but I am confused about that? We arent allowed to use medical diagnosis as the related to's. Also you cant use acute pain related to chest pain, that doesnt sound right at all. I am just so confused right now normally I am good at this but I am just drawing a blank. If anyone wants to help a poor nursing student I would greatly appreciate it! :chair:

I LOVE Grey's Anatomy, already bought season one on dvd, waiting for the second one to come out. I usually dont miss it though. Will have to get my dh to record it this season for me. I also watch discovery health channel as much as possible, I love the mystery diagnosis and medical mysteries. All of them are great though, I like house and er as well. All in all I have to say Grey's is the best show ever.

I am starting at Southern West Virginia Community college in August. From what I have heard its the number 1 ADN program in the state of West Virginia, I am really quite excited that I got accepted. I got in on my first try! WHOO HOO!!! I cant wait.
