1) What kind of program are you in? What is the test for?
2) What came before that question? What are you being asked? I mean, we can define all those terms, but I'm not getting where you are confused so I don't know how to help you.

I think she is asking when given certain scenarios
. Such as vitals- assessment, giving a pain med- implementation, patient's response to pain med- evaluation.
Oh, OK. So if she knows the definitions of what those terms mean, she should be able to see if there is something about them in a given scenario?
If this is the case, OP, where are you confused about the definitions and what they mean related to patient care?
im sorry yes I was asking about whats the way to remember these when taking the NCLEX PN test. I cant sit there under all the stress and try to remember all of this stuff. Don Thank you for your answer. I hate timed test I freak out over it. I dont like to be under pressure like that and I freeze. I failed my first NCLEX and im trying to get back into the swing of studying and I just dont know what to study so im just studying everything and im all over the place doing it. If anyone could help I would be one of the happiest people on earth.

OP the FIRST place you need to dart is what portions of the test you were below passing;
Next, you need to review questions and rationales in a NCLEX format style, meaning, "mock" the NCLEX, and go the distance...85 questions, 105, 165, 205 to give you a sense of normalcy in getting the amount of questions to think critically.
Next review answers and rationales; also, pay attention to your scoring...is it near 70% or more? The higher your percentage, the higher your chances of passing the NCLEX; that's why reviewing your response in choosing the BEST answer by thinking like a nurse is measure through review and readiness predictors; and reviewing what you missed.
Sending positive vibes in combating your anxiety, and succeeding the NCLEX-PN.
