How much does a home Health nurse make ? Is it by hour or patient ?

Traditional intermittent skilled visit is usually paid by visit depending on level of care. You see multiple patients per day.
Private duty home care where you work a 4,6,8,10,12 hr shift work is generally paid hourly.
Rate of pay varies by area of US. The nurses level of experience, LPN or RN, additional credentials such as oncology, high tech IV, trach-vent, etc.
California expects agencies to only hire nurses with a year ( or more) of full time nursing experience. Agencies that accept Medicare are not supposed to hire new grads

Agencies (employers) will typically gauge their hourly pay rate in some relation to the rate of reimbursement for the case. That is what they will tell the employee. You can't expect to receive , say, $45 an hour, if the reimbursement rate is, say, only $27 an hour.

It depends on a lot of things. For visits, per diem staff at my agency are paid per visit. I am full-time so I am salaried. I get per visit pay for any visits done outside of my salaried schedule. I think most agencies pay their full-time visit staff on a salary as opposed to a per-visit basis. I have many former hospital co-workers working permanent positions for various VNAs and none of them are paid per visit. There are some large national agencies with lawsuits pending because of per-visit pay.

All agencies I have worked for, and there have been quite a few, are pay per visit and it is a flat rate. There is no compensation for increasing complexity of visits. Visit to patient with DM and HTN is paid at the same rate that visit to patient with wound vac dressing changes. Whether the visit lasts 30, 44 min or 2 hours the pay is same. Acuity is not figured into the system. So glad I am 98% retired.
