I never let my personal and professional lives cross and never was in trouble professionally, but went to rehab for drinking too much wine to cope with certain things in life. So that is in my medical history. I am in NP school and will be finishing in a few years. Has anyone been in my situation and applied for the NP license and had to be monitored? Even after being sober for years? I know they ask this question on the licensing application.

Here is how it worked with me- I didn't have a problem until I did- and I did cross that line. Then the board asked me to voluntarily release my medical records...
Now I didn't have anything to hide... but the MD they didn't know about they didn't get the records... so depends if there are any cross referencing records in your records.
I have heard from many people, that to disclose this information is a likely enrollment in the monitoring program with all of its restrictions and drug screens. This would hobble chances of getting a job.
To me this is your private health matter, and these questions are not relevant for the one size fits all diversion programs.
So you marked yes to having alcoholism? I don't graduate for a few more years and it hasn't hindered my RN license because I haven't had to answer that question when renewing my license. This totally sucks. I don't want to lie, but it's like I went to rehab, I did my part in getting better, and Im doing what I need to do daily to stay this way. I don't want to be in a monitoring program or bring this up and make it into a whole to do and hinder my job chances and make a "black mark" on my NP license when there doesn't need to be one. You know?

When I renew my NP license I am not asked any different questions then for my RN. Is that the concern?
I'm a RN now. My concern is when I graduate and file for my first time to get my NP license. I've looked at the applications in my state and the state my husband and I may move to ... My state asks if you've had a problem or been to rehab for drugs/alcohol ... The other state asks if it's been in the last 5 years... I'll be just shy of five years since my rehab stint by then
The initial application asks these questions. My RN renewal did not ask this question.

Don't lie on your application. The one thing all boards have zero tolerance for is someone who lied on the application. With that said depending on the state many will put you in monitoring just to cover themselves. If in fact you will be close to five years when you graduate simply wait that short time. Sign your application one day after five years and you have no worries.

I would not disclose that. Period. If you are really concerned the best person to ask is a lawyer.
