Does the following sound correct: A 26 week preemie, 1 week old, 800 gms., inside an isolette, swaddled on air control with humidity at 40%.

We've never used humidity with an infant who was swaddled. We are only currently using humidity with our Giraffe incubators (except mist tents for a few days on the micro-preemies). We generally only use air control and swaddling once on infant gets to around 1400 grams though often earlier if they are IUGR and "active" (agitated).

Uh, no. Why would someone swaddle an 800 gram one week old 26 weeker in the first place? And leave humidity on? The only time we leave humidity in the bed past one week is...bad skin, extubated with O2 to the isolette and a raw nose from CPAP. That kid doesn't even fall on an NTE chart anywhere. All he is doing is using more energy to stay warm.

No No No. While I have switched a baby that size to air mode on occasion, he needs to be on a skin probe to meet the NTE. Humidity is usually used on the smaller babies while they are on skin control. There's also the risk that his swaddling and clothes will get moist and keep him cold.

In Sweden all preemies under 32 weeks generally stay in isolettes for their first couple of weeks. The earlier born the more humidity and ofcourse higher temperatur in the isolette. At my ward all babies under 30 weeks starts with 85% and every two weeks we set it lower and lower (70%, 60% and finally 40%). When they can keep warm at 28 degrees celcius and 40% humidity, it´s time to move out. They stay in isolettes regardless of cpap, vents. It seems like that´s a big difference from how you do it in USA.
