hmmm....well i am not the best at this and doing it without a calculator----i get 25 cc hour....you have to double because of the concentration of the dose on hand...does this sound right

i get a rate of 10.....and dont understand why they are asking for VTBI

when no end dose is stated....

order: diltiazem 125 mg in 100 ml d5w at 10mg/hr iv

find the rate and vtbi:

rate: ________

dose desired: 10mg/hour

dose on hand: 5mg/ml

10mg/hour x 1ml/5mg =
1ml/hour
(patient is getting this much of the
diltiazem solution
per hour)

vtbi: ________

10 mg
(dose desired)
/125mg
(dose on hand)
x 100ml
(amount the dose on hand comes in)
=
8 ml
(vtbi--this is what the iv pump will actually be set at)

so basically you want 2ml/hour to equal 10mg/hour.

im confused why would you only give 1ml an hour when the vial is 5mg/ml. and the order is 10 mg/hour

This is an intensive care medication given for SVT and atrial fib. It is often titrated and given in very small amounts, goes by the dilution of the diltiazem in its diluentant and then resulting IV solution which is usually a small piggyback bag.

I know that it just seems the dose should be .1 ml an hour instead of 10

i, too, know what it is....but the volume of the concentrate is so large compared to the volume of diluent, doesnt it need to be taken in to account? having not worked ICU, i have never seen a pump that would be set up that way....to me VTBI is set to deliver a prescribed dose....not a rate....which is accomplished otherwise......

im confused why would you only give 1ml an hour when the vial is 5mg/ml. and the order is 10 mg/hour

the 1ml/hour refers to how much of the drug is actually being given. look closely.

10
mg
/hour
is a different expression than
1
ml
/hour
.

this is how this drug is dealt with when given in the icu and ccu.

this problem is dealing with 2 things: (1) how much of the diltiazem is being given and then (2) how much of the d5w is being infused.

this stuff is, as the op first said, confusing. it helps if you were standing looking at the physical situation.

something confuses me here, if the availible line states you have a vial of diltiazem containing 5mg/ml are they not implying that you are adding x number of ml of this vial to the bag of 100 ml of d5w. in which case it would be 105 ml of fluid to be infused? (5mg/ml= 25mg/5ml) or am i just tired....
