GIP Computer Charting

We were just told that we have to now start putting IN & OUT timeswhen charting patient care nursing notes. We were told that it is aMedicare requirement.

I'm not sure I am buying the Medicare requirement thing. I have searchthe internet & the medicare site looking for this requirement.

[COLOR=#000000][/COLOR]I thinksomeone in the office is trying to make their life easier by making mine 10xmore difficult.

I've been told the same. Not sure if it's true or not but why do you feel it is more difficult?

I have seen it in the COP's but can not recall where....I hate when that happens :)

Try a search in CMS/hospice/regulations or check the Federal Register....also, check the Open Door Forums on the CMS site.

Is the GIP charting in reference to a hospital computer charting or your hospice charting program?

Our in and out times are captured in our hospice charting program. We also document a concise note in the hospital charting system, but it does not have times documented. The hospice charting program is where all of our regulatory requirements are documented, the hospital charting is more of a communication note. The hospital note does include s/sx being addressed and that the patient continues to meet requirements for GIP level of care.

Is this a GIP requirement from your agency or a documentation policy in general?

In my view this is simply an "interpretation".

I am not against including a comment about the actual time spent at the bedside in the way that other professionals might.

From a practical POV however, many hospices do not provide their own GIP nursing care, it is provided in a hospital or in a SNF on many occasions. In those instances the hospice can clearly NOT dictate their documentation policy based upon a "notion" that cannot be fleshed out in an actual CoP requirement.
