Still need more details. If surgery is that emergent there's no time to waste tracking down the next to next of kin.

You forgot the most important thing: your initial thoughts on the issue. It's of no help to you whatsoever if we just give you the answers right off the bat.

And think about a whole patient approach. Can this patient consent to his own procedure? Because if not, there may not be a procedure. Why would you involve his family--you would need consent for that as well. And you need some sort of initial report on the patient--but I am not sure from the OR....as apparently he has not been to the OR as of yet.

If you have more details you care not to share--use your resources, do not take all this on yourself, you need to figure out who if anyone can make decisions for this patient if the patient can not, and most of the time facility policy decides who to go to with this. Usually the house supervisor.
