AVC RN program fall 2019

I got my acceptance today!! So excited!!

I was looking through the packet today. I got the background check started and filled out the forms that are due by the 24th. Did anybody else receive the name badge requisition form, I didn't get it in my packet and it's due by the 24th.

No I didn’t get that form either.

Says it can only be purchased from the SLO office after semester begins and you have to be instructed to do so by instructor.

Unless I’m looking at something completely different ?

Thank you! There’s still hope! Mail might be late for you too! I had emailed the department on Friday to see if they send out letters to those who don’t make it in this round..so if I get an answer from them I’ll post on here. Good luck!

Yes! Thank you! It’s been hard trying to get a hold of anybody lately. I would really appreciate that.

Do any of you know what we are supposed to put for the “Department/Company” and “Title” section of the Attachment A form?

Hi :)

I just spoke with someone from the Health and Sciences department and she said that they had a policy change and the name badge requisition is now a form that we will receive and complete with our instructor.

We are supposed to write “Antelope Valley College” under department/company, “student nurse” under title, and the first day of the fall semester as the effective date ?.
