VisaScreen Application

Hi! I'm an NCLEX-RN passer in the State of Illinois, but I'm still in the Philippines. I'm a bit confused about the application for visa screen. Here are my questions:

1. How do I request for Validation of license/registration to IDFPR? Should I send them an accomplished copy of the validation with the $20 draft?

2. Which VisaScreen category should I check?

3A. This is silly but I just want it to be clear, um.. Regarding Education, Secondary school is high school, right? Then Tertiary is college? 3B. After my Bachelor's degree in Nursing, I went to graduate school for Master of Science in Nursing, should I send my Grad school a request for academic records/transcript too?

I hope someone can help me... Thanks!
