Back In 2014 I had titers drawn for nursing school. Came back I was not immune to HEP B. Got a series of booster shots and a redraw. Came back I was now immune, great ! 
This past July got another set drawn for different job. No issues 

I just accepted a contract with a different agency and needed them done again. The results just came back and again it says I am not immune to hep B. Has anyone had this happen ? Now I am worried I will not be able to work this contract

Yes I've had it happen.  From my own personal experience, hepatitis B immunity can wear off over time.  I haven't researched any studies, but I believe it does because I was immune for several years and then 'boom' I wasn't.  The employer gave me the series over again and still remained non-immune.

I have had the Hep B series three times and still not immune.  My last employer gave me a form letter with all the dates of the vaccines and titer info to have for future reference and basically told me not to bother with the vaccine anymore.

I had this happen with the MMR which for some reason doesn't take in my system. Every time I take a new job or start a program I need to get titres. I'vw been vaccinated 5 times in the past ten years.
