I don't know if this is quite what you're looking for, but I for my "final" paper in order to get my nursing degree we had to identify a problem in nursing and write about it. My topic was regarding New Grads in the ER. The answer to the critical care nursing shortage? Train new grads... BUT how are they being trained and are they being trained effectively? And what are the new grads views on being in the ER? I actually found A LOT on this topic (mostly from Canadian nursing mags) and was able to make my own recommendations.... enough for a 20-25 page paper. You might be looking for something more to do with actual ER patients, I don't know....

How about how to manage repeat visitors to the ED - chronic pain, etc. and treat more quickly/effectively? For that you will have to review current processes in your ED, pt populations and find ways to troubleshoot.
Another issue could be security of medical staff in the ED - a biggie we are dealing with right now.
Also, in our ED (assume it is similar everywhere) abdominal pain pts take up an enormous amt of time. You could focus on these pts, why they take so much time to treat, and how to manage the flow of these pts through the ED to dx.
IMHO most major problems in the ED (this according to mgmt.) are surrounding surges (influx of people at certain times), treatment times and bed control, pain control, and pt. satisfaction scoring etc. Nurses at your specific ED might be able to provide more specific issues.
Good luck!

Computer vs. Paper charting
5 stage vs. 3 stage triage
Does having pediatric nurses treat peds in the ED lead to better pt/family satisfaction?
Use of EMLA/Lido/NS before IV starts
Development of protocols (abd pain, chest pain, etc.) and their influence on wait times
Utilizing social work to curb frequent flier visits (or trying, at least)
Use of pt. advocates for pt. satisfaction
Which method of report to floors/ICUs works better for decreasing holding times while also satisfying floor/ICU nurses?
Development of holding units and how to staff them (floor nurses come down? ER nurses staff? Registry?)
I used the charting question and found a ton of stuff. The other topics are things other classmates used, or things I would like to research for my ED. I may still have my paper... if you need help, pm me.
