Shift Report Organization

This semester we are required to take report on all of the patients (not just our assigned patient) and I was wondering if someone had any ideas on how to organize this information. The reports are taped and I would like to have some kind of template/form to record the info as it is playing. any suggestions? Thanks!

These sheets can document up to 4 patients. I suppose you could copy it on both sides and have up to 8. Hope it helps :)

My report sheet for 5 patients. I used to have one for the nursing home I worked in that had room for 16 patients, but I lost the file when some computer geek erased everything on the hard file of my laptop :devil:.
