Help!!
I am writing my care plan and one of my nursing diagnoses is Risk for Sexual Dysfunction r/t changes in hormone levels. This is for a post-op hysterectomy patient. I need to have a long term and short term goal with time periods (ex: by discharge, 1 week post-op). What are some examples that I could use for these goals?

Are you sure this is your prioity diagnosis? What about the pt's post-op pain, risk for infection, impaired skin integrity, etc. All of this should be higher in priority while the pt is still in the hospital

I really think you need to look for another diagnosis. Her sexual health is very important, but it can't really be addressed until she is ready to resume coitus. Is she is having her ovaries removed? You could develop a care plan that talks about sexual counseling, but it really isn't relevant for the time frame you are quoting.

What makes you believe this patient is at risk for this and is this the most important rick factor this patient has at this time in the immediate post op phase?

BTW, I think it's perfectly OK to look ahead into the future for your patient and include a 6-week or 6-month goal if you have determined that those are appropriate. Well, what do you think, and why? We who have had hysterectomies might have some good ideas for you, but you should be able to find out a great deal about this in your reading. So why don't you tell us what you know first, and them we can chime in.
