i'm worried that my recommendation letter will get removed. it has already been submitted; however, if it is removed, do they usually notify the applicant?

removed.....how? If it's a requirement for the school to which you are applying, and it isn't there, I imagine you'd receive notice that your application wasn't complete. Most applications are done online, attachments submitted either electronically or by separate cover....so not sure what you mean here. If you can be more specific, maybe we can help you better.

You can ask to check your file. The person will either let you see the contents of the file, or they will look at the file and tell you what documents are in the file. I had to do this one time and the individual cooperated with letting me know what was in the file without removing anything.
