Most programs will require a BSN for entry, or some other type of Bachelor's degree for admission as a Nurse Practitioner is given a MSN when they graduate from the program.
Depending on ths state where you live, NPs can and do open their own office for treatment of patients. However, with peds, you will see most of them employed by hospitals or working in tandem with a physician. Many pediatricians now work in group practices so that they can get some time off and not be on call every single nite. Again, it depends on where you live, etc.
