I asked one of our inpatient physician. he told me that hospital provides physician the link called antibiogram for antibiotic. In this antibiogram, it will tell physicians what antibiotics treat what bacteria and it also depend on the communities I will work with. From there, I could prescribe the antibiotics.

Hi everyone, I am NP student and wonder if anyone could help me with antibiotics guideline in reality. My story is that I shadowed my preceptor, who works at the clinic, and one of our patients who presented with increasing frequency of urination, dysuria, and microhematuria in urine via urine dip. My preceptor diagnosed her as cystitis. My preceptor prescribed Keflex 500mg twice a day. I asked the etiology of prescribing Keflex instead of Nitrofurantoin per CDC or uptodate recommendation, but I could not understand well what she was trying to explain to me. Does anyone could light my mind up? Thank you

thank you for all your input. How can i check the policy of the facility? I don't want to ask my preceptor that giving the IV without the pump is in the policy or not. my preceptor said not all the antibiotic can run without pump such as Vacomyocin needs a pump.

Good morning everyone. I am a new nurse in nursing career and have so many questions but one of the question i would like to ask the most is that in Nursing Home facility, my preceptor told me that the some of Antibiotic such as Cefazolin can infuse without IV pump. she said I can adjust the rate or the drop by using roller clamp. It was just basically like a cram of info with basic drugs and positions and infection precaution pneumonics that really helped me memorize things! After having to spend $300 on each time I took the exam, I really didn't have the money for a personal tutor or paying for another review. I really budgeted. i did report to my nurse and elevate his leg during my shift. the main purpose i posted this topic is that i can find the answer for my curious. In my response, i said it was my opinion. I believe student nurse or even experienced nurses sometime we do not know everything, so we have to give hypotheses and try to find the good answer which fit the best. That is how i learn and this is a dicussion forum and i want to share what i am thinking as a student nurse level. i did not mean to doubt on anyone's compentency.

Gout, cellulitis' sign is red in the affected area but he did not have redness on the right foot and his temperature was 98.7 F. One thing is cellulitis only occur if he has a cut right? but his foot's skin was intact.

Thank you for your info. In my opinion, it could not be any of them. First, he has been in the nursing when i worked in the facility (almost a year) and he never has any edema. Second, gout usually occur at the fingers (swollen, red and painful compared to his sign which is foot edemaous and painful). Moreover, he never has gout in the past. Last one, from what i know, cellulitis signs are red, swollen, and painful. It usually occurs at lower leg, not foot.

he has hemoparesis for long time. why does it happen now? if it is edema, why would it cause excruciated pain?

I am working as a CNA in nursing home. Yesterday, when i took care one of my resident who is paralyzed right side ( cant move right arm (RA) and Right foot (RF)), i found out RF was swollen (+4) and LF was normal. it was excruciated when i moved it. it was not red but warm ( because previous CNA put the sock and cover him with blanket). I reported to the nurse and the nurse told me to elevate the affected leg with pillow. Can anyone explain why did he have edema in the paralyzed foot (not for unaffected foot)?

Can you explain more about the way they wash the blood? if you watch the video, what does that procedure call?

thank you for all of you. I was reading the Nclex book by Saunder. One of the chapter it says about washing RBC is a procedure to remove the tissue debrie in the blood. However, it did not go detail about that. that is why i posted this topic and hope someone can answer my curiosity. i looked up on Youtube and one of video talks about washing RBC. For me, the procedure looks like dialysis. there are 2 ports that insert into patient's arm. One port is patient's Fatty blood goes through a machine, then comes out with bright red color into the other port. here is a video

That is procedure to remove the tissue debris in blood out and reinfuse the new blood in at the same time.

I know that washed RBC is the process of removing debris or dirty blood inside the body. I tried to look up the websites about when do we need to do it? Who can be qualified to do it? but nothing came up to answer my questions. I've never heard about washed RBC before and it makes me curious to find out. One more question is that where can i observe the procedure? I am appreciated to anyone can fill in the blank for me.

let me clearify what i understand about your saying. afterload is the pressure in RV or LV during systole to overcome the pressure of Pulmonary veins or Arotic so that the Pulmonay valve or Aortic valve will be opened and blood is ejected to Lungs or body and close to prevent to backward to RV or LV due to the pressure of RV and LV decrease after ejecting the blood, right? Am I right that Preload is the pressure of RA or LA to overcome the pressure of RV or LV to open the AV valve so that blood can pass down to RV and LV?

I believe that the difference between FFP and platelet is that FFP does not contain Platelet but clotting factors. when you labeled platelet can confuse other people with physician's order.

One of my classmate will turn in 58 when he graduates from nursing program. he shares his story in front of the class on the first day in nursing school that, " I never give up on my dream even though it takes me 5 years to get into the nursing program. if tomrrow is my last day, i will still try my best to achieve my dream." i hope this real story helps you.

i suggest you wait until you get into the nursing program. it depends on your school. my school is that i can take CNA test after 2 quarters. dont wait your money on CNA program. Moreover, you can apply as a student nurse or nursing assistant in the hospital without CnA license when you get into nursing program. Good luck.
