  1. We had an older lady for a patient on our floor that in report, I was told was hallucinating....was told she was seeing people outside her window at night....we are on the 6th floor....anyways, I went into room to do assessment...she seemed pretty with it...knew all the answers to my questions....she also mentioned the people outside the window to me....I assured her that we were too high up and walked over to window to show her....about that time, I looked over to the left side and you could see the nurse's station in the annex of the building...so she was seeing the nurses in there....so i asked her if this was what she had been seeing...she said yes....so turns out she wasn't crazy...
  2. It is the LPN's responsibility to let the RN know what IV meds she needs and times.....where I work, the RNs have their own patients to worry about...no way they can keep up their 6 and mine too...that was the LPN's mistake..not yours.....
  3. I am on light duty....not allowed to do patient care...I put in orders, chart audits and answer call bells.....I am not factored in staffing, so I get to work and nurses get extra help....

In my opinion, you did right.....unless someone is having an emergency.....you were already in process of giving medication to first patient....just say you have 3 or 4 family members doing this...you would never finish med pass....I had a family member do this 1 time...I kept stopping to attend to things like reposition head, change pillow...turned 2 hour med pass into almost 3 hours....family member returned the next day...while she was there I gave all meds to that patient, breathing treatment, reposition, and anything else she asked...I then started my med pass...had not got to first patient before she coming out of room...I asked what she needed....pillow moved...I very politely told her that I had 29 other patients waiting for their medication and would get back to her as soon as possible....she literally stood at door and watched me do my whole med pass....in meantime, the tech that was on duty was literally in and out of room several times...I think it actually took her seeing me working on my feet and sitting charting to realize I meant no harm....after that she was still needy but made sure to get all needs met before i left room....of course this only lasted a week, while other family members were there....then she was in and out in 15 min...and seemed quite happy with care....
  5. I work on postpartum nursery. We have 2 charge nurses. The ideal situation is that 1 takes the floor which consists of 30 beds and the other oversees the nursery....that is the ideal world....what really happens is that 95% of the time both charges are in the nursery going to c-sections, working on sick babies and helping out however they can because we are shorthanded always....when we have floats, we will get an admit and the first thing they do is look for the charge nurse to do orders and assessment, doesn't matter if LPN or RN....when told our charges don't have time, they ask why.....they say that is their job...what they don't understand is that babies are patients too and that you don't have the luxury of puttting them in a room and get a set of vitals until the charge gets to them.....It never ceases to amaze me how many RNs are incapable of doing an admit assessment....I am a LPN...worked nights 3 years...no admit team....shorthanded....learned to get admit, do assessment, have Charge RN check behind me, orders in computer, and chart opened in an hour....have worked days now for 2 years....I appreciate when I float....the other floors Charge nurses actually do assessments, put in orders, and help out as much as possible.....I respect my 2 charge nurses....delivering babies, taking care of sick ones, assigning rooms and sorting out messes that invariably come up on the floor....You could not pay me enough to be Charge....takes special nurses to be able to get everything done and still have braincells that work at end of 12 hours. SN...we do not have unit clerks, 6 patients apiece....sometimes an aid and sometimes not....we get a ton of medsurge overflow....most times you have more medsurge than postpartum

We had a lady call the ambulance.....said she was in severe pain from being constipated...they started to examine her and discovered she was in labor....when they told her...she denied it....she denied being pregnant all the way to the delivery....finally the doc said fine....you just delivered a 7lb turd....she finally believed when she saw the baby...btw...this was her third child.... The next one was just a few weeks ago....lady came to ER saying she was cramping and couldn't stop peeing on herself...she was in labor and swore she did not know she was pregnant...also told she had just snorted a gram of cocaine...fortunately, as far as we could tell, the baby was ok....we sent her home 2 days later...we discovered she had left her drivers license...called her to inform her....she did not even remember what hospital or room she had been in....
  8. You should have reported as soon as patient told you....I learned this the hard way too.....things have a way of getting blown out of proportion and being worse than they should be.....but we all live and learn....as I tell my orientee.....you will never make this mistake again......
