  1. Hello, I will try not to make this as short as possible. I am a 101 student and our first full case study assignment was just to find someone older than 65 an come up with a care plan for them. This is not for a grade, just for feedback. I chose my MIL who is 71. My priority nursing diagnosis is: Disturbed sleep pattern RT frequent awakenings amb waking six or more times per night, patient states I am so tired, I haven't figured out this new c pap machine but I hope it's going to make a difference” I am having difficulty coming up with a secondary diagnosis. She has a plethera of other medical conditions, but all are currently under control. This is her history: Hx: Paralytic Polio age 7, multiple surgeries as a child, left leg paralysis, wears brace, ambulates with crutch Partial hysterectomy 2005 Cholecystectomy 2012 Fatty liver disease Increased cholesterol High blood pressure/leaky valve Sleep apnea/ c-pap machine Stress incontinence Hx of falls (Shattered knee caps 1992) Anemia Hypothyroidism Before coming up with our primary diagnosis we had to come up with four. These are the four I came up with based on her history and assessment: 1. Disturbed sleep pattern RT frequent awakenings amb waking six or more times per night, patient states I am so tired, I haven't figured out this new c pap machine but I hope it's going to make a difference” 2. Risk for falls RT neuromuscular impairment amb use of crutch to ambulate, brace on left leg, hx of falls. 3. Stress Urinary Incontinence RT loss of pelvic muscle tone amb leakage of urine with coughing, laughing, sneezing, wears poise pads. 4. Impaired mobility RT neuromuscular impairment amb use of crutch to ambulate, wears brace on left leg. Even though she has a history of falls and impaired mobility, I don't feel like those are her current or primary problems right now, but I can't seem to get together anything else to go with my first diagnosis. Can you please help? Should I just scratch it all together and go a different direction? This is my ob and sub data from focused assessment: Pleasant disposition. Pain of 0 on scale of 0-10, T-98.6, P-64, R-16. BP 138/80. Ambulates with assistance of crutch. Wears brace, left leg paralysis. Steady gait with assistive devices, good posture. Complains of tiredness. States I haven't figured out this new c pap machine but I hope it's going to make a difference” Sleeps 3-6 hours a night, is tired during the day, has two cats that wake her early in the morning. Does not feel well rested. Frequent yawning during conversation and dark circles present under eyes. Thank you in advance
  2. Anxiety is sky high! But good anxiety. I hope ive read enough. I am so confused by the calendar/D2L/Course Point my head is spinning! I am sure after the first two hours of introductory I will understand better. See you guys in the AM.
  3. I really enjoyed the seminar yesterday. She gave so much good information, and was so funny. Did you guys go?
  4. I do not work in a LTC facility, and i am not yet a nurse, but I am a nurse tech on an IICU floor and I can tell you it is insane. We have 16 patients (beds are always full with patients waiting for admission). There are 4 nurses and 2 techs iF we are fully staffed wich is rare. Generally we have 3 nurses and 1 tech or no techs because they call out a lot. Most of our patients are total care, but either way we are a step down unit so there is soo much to be done. As a tech my job is to: Prepare rooms for new patients Transfer or discharge patients Vital signs every 4 hous for every patient or more often as requsted by the nurse Document intake/output every 4 hours turn patients every 2 hours Insert/remove foley caths Glucose monitoring for all diabetic patients deliver diabetic snacks Transport and return patients from dialysis Bed bath for every patient every day (if paitent refuses must try on second shift) Change linens every day ambulate patients or transfer from bed to chair or vice versa feed patients take and pick up specimens to and from lab assist to bedside commode or bed pan answer call lights anything else requested by nurse staff Our floor is crazy busy. The time passes so quickly it makes my head spin. I am lucky that my floor has computers in each room but i have to sign in and out and document every single time i am in a room. Things can also be slowed immensely when we have multiple patients with C diff, hepatitis or some other issue requiring dressing in and out of a room. I get behind on most days. It seems impossible. Sometimes by the time I am done making my rounds for vitals, Its time to start over! It literally takes 4 hours to get through 16 rooms. I can not imagine having 25 patients, and I am just the tech! I imagine that the nurses at this facility feel that the patient load is way too much. I dont know if they have techs or nurses aids to help them, but without them I would think it unrealistic to think a nurse could care for that many patients the way they would like to.
  5. Hey. I was just looking at my calendar and I put the Success in Nursing Seminar down for Thursday the 13th. For some reason I thought it was on Friday. Can you please tell me if I wrote it down wrong or if it is actually on Thursday??
  6. we have to have a patch on the uniform and the lab coat right?
  7. each school is different. at midlands we were not allowed to use calculators. I dont know about cctech.
  8. No kidding! I went yesterday to pay my tuition and apply for a loan. I didn't have time to get uniforms and name tags. I get so lost in Sumter! I am going for my ppd test today and physical. I am going back to sumter this week for books/nametag/uniform. I thought I would wait until after the 5th since they wont have parking decals until then. I feel super stressed trying to get it all done right now. I feel like I am forgetting something. I just want my books so I can start reading. Do you see how much we have to do in the first two weeks? and our first test is on the 27th!!!!!
  9. oh well, I guess I didnt make the cut. Boo. lol.
  10. Jamie did you get the book scholarship? I never heard back.
  11. can you give me the link to the one on all heart? I dont know what the uniforms look like so I was afraid to go online to purchase. Jamie what thank you letter? Is that something you are doing or something we are supposed to do?
  12. I am going up to the school tomorrow to get my parking sticker and purchase my uniforms/books etc. I got an email on my cctech gmail that says all payments are due by Aug 3 but my financial aid wont be in until Sept 3. Do I have to pay for everything out of pocket until my Financial Aid comes in? I am freeking out. Books and uniforms and physical and everything is over $1000 and my financial aid doesn't cover any of that. I don't know how I am going to pay tuition to hold my classes???
  13. I put my career on hold for the last decade. I had no idea what I really wanted to be so I welcomed marriage and children. When I finally figured out that nursing was it, I was 28 and I had 3 children ages 1-5. I went on to school, but I didn't make it through. I was terminated from the program and no other program would allow me to apply. I was devestated to say the least. BUT, I really, really wanted it, and it just so happens that another school here recently started a forgiveness program. I am now 38 and it has been 3 years since I was dismissed. I was recently accepted into a different program and I start my clinicals in just a few weeks. I believe, where there is a will, there is a way, and I plan to show my children that they should never give up on their dreams. You may not be able to go right now, but that doesn't mean your dream should be dead and buried. If you really want it, it doesn't matter how long it takes to get there, only that eventually, you do. Good luck on your journey.
  14. yes. White socks or support hose.
  15. Can you tell me how much the uniforms were? Did you get your lab coat and patches too? My financial aid doesnt cover uniforms so I am trying to figure out how to pay for them.
  16. I went to the LAbcorp in Lexington and I was a walk in. I just took a copy of my email. Was in and out in 15 mins. I was out of town last week so I wasnt online at all. Were you able to get the book scholarship form Jamie? I sent you a PM too.
  17. That was me with the scholarship form. I did receive the form. The Finiancial Aid counselor was nice enough to mail it to me since I live so far away. I already paid for my background check and did my urine screen today. I still need my physical and titers and ppd. Also I need to set aside a day to drive up to Sumter to order my uniforms and nametags. I am planning on attending the success in nursing school seminar too. I haven't bought my stethescope yet because she said they were giving one away as a prize. Just in case I'm lucky that day. lol.
  18. OK. I see it now. I think if it is listed on the acccount summary then it is included in the tuition and financial aid will cover it. Items not listed, like uniforms have to get permission to use financial aid, or paid out of pocket like the background check etc. You can call financial aid to be sure. They went over everything that would be covered with me so I could decide if I should take out loans.
  19. I know the ATI program is included in the tuition. The NCLEX review and fee to take the test is listed under second year, so I am not sure if that is part of the tuition, or something we have to sign up for on our own.
  20. i took my CPR class on Wednesday. I highly recommend the instructor I had. I signed up for the class through the AHA website. The class was $75 and it was four hours. She gave me permission to post her information. Her name is Theresa Hancock and she teaches at the Outback Saftey Training site in West Columbia. If anyone is interested in taking her class I can pm you her number or you can go to heart.org and find a class near you. Also, make sure you sign up for BLS for Healthcare workers.
  21. Sir, for the last time, you are not dead or dying, and even if you were I can assure you that, this is not the 'light' that 'They' are referring to!
  22. I was just reading back through the comments. I just wanted to forewarn you that the schedule online is not necessarily the schedule the nursing program will go by. They will add stuff all the time, like the ATI orientation. You will go by the schedule THEY tell you. That is why you can not work while in nursing school. Well, that and the amount of time it takes to study. Not trying to scare anyone, but it can be intense at times.
  23. yes. once you pay you will receive an email with a location neer you for the toxicology screening. you have to print the email and take your drivers licence with you for the urine screening.
  24. No, you don't have a choice. You have to wear the ugly standard issue uniforms. lol. But they sure look better than the one I had at Midlands. At least these dont have an ugly collar. I opted to take a local CPR class this Wednesday. Its the same price but closer to me and only a four hour course as opposed to an all day event. I also set up my background check and toxicology screening. Next is to get my uniforms in order and figure out how I am going to pay for my books. The financial aid office is mailing me a scholarship application for books. I really like the Financial Aid staff there. Everyone is so helpful and knowledgeable. Its a complete 180 from what I experienced at my other school.
  25. Hi Ladies! What did you think about orientation? That was a lot of information, I feel like I have so much to do. I need to renew my CPR Certification, (Mine expired in March). get my 2 step, get my records of my other shots, drug screening, back ground check, Uniforms, name tags. I wish it was all in the same place. I had to contact financial aid because there was a block on my account and I couldn't register for my classes. They fixed it for me but I found out that even after my grants I will still have to take out loans. My aid only covers the actual tuition, not books, or uniforms or even the ATI account. Im pretty bummed about that but I will find a way some how. Are yall planning to go to the Success in Nursing Seminar?
