Hi everyone, I am also a brand new nurse, and I just started doing home health, and I'm having a hard time charting and documenting. I graduated a while back, but I just recently passed my boards, and a hospital job is a little tough right now to get in, so I kinda landed in home health care. I even had to volunteer to shadow one of the nurses a few times, but she didn't really explain how to do it. Now I have all these 3 cases I opened last week and am still not able to complete my notes. Can anybody please help me? Can you guys send me some examples of comprehensive assessments and some follow-up visit notes? I would really appreciate it.

Congratulation for passing ila and opi exam .. i am also in the process of endorsing my License from California to maryland and it looks like the easiest way to do it is to take the ila, opi test instead of the IELTS, TOEFL but im just wondering how did u guys prepared for the test?

i just took the nclex rn last monday ..everybody is different your the only one can tell if you think it would help if you unwind 2 days before your test or use the remaining time studying for me i used all my time to studying even the last day . it works for me . the only thing i can advice is try to get some good sleep and nopt to eat a lot for breakfast because you dont want use your energy trying to digest your food . and before you start your test pray ...pray and pray that god will guide you all the way..." trust in the lord with all your heart and lean not into your understanding, in all your ways acknowledge him and he will direct your path" Good Luck to you...

Leo, where u from? because i took excell nursing review i have been out of school for 14 years and took nclex 3x and failed so i decided to take core content review and they are really great ... i took my nclex again last monday and my comp shut off at 75 did PVT and got a good pop up ..now im just waiting for my official result..

Hi travis, just wondering if your name appear in BON yet? just checked mine and still not there ,... also BON its not updated with todays date yet im hoping to see my name today but then again u said they were close yesterday .. i actually tried to contact them but no one answered so probably they were really closed ...im starting to panic and doubt PVT really works geez this waiting is killing me ...

why dont u try PVT ? it makes the wait a little easier.. i took mine last monday at it shut off at 75 as well , i wouldnt say its easier from my previous attempt but i was kinda glad it didnt go all the way 265 , i had a lot of SATA maybe around 15-20 ,1 arrange in order , one exhibit, lots of priority( who you going to see first) and lots of medication ( which meds would you question?) ang lots of infection control (cohorting) and some psych and Endo i didnt have match question w/c i thank God bec im so bad at math . when my comp shut off at 75 i started crying and told my self probably i failed again ... so when i got home i wasnt going to do PVT again but couldnt help myself not do it so i went ahead and did it.... glad i did because i got the " good pop up" couldnt believed it. im still waiting for my name to get posted at CA BON .. but im saying is go ahead and do it ...you never know ...u might get the good pop up and it will your anxiety go down .... be positive but be ready for the worst ..its not the end of the world if u didnt pass just need to get yourself together and do it again... im telling u this bec this is my 4th time taking the nclex and i have been out of school for 14 yrs but i didnt give up ... but this time i took studying very seriously i even have to resigned my job just to focused . i took excell review class in san deigo last feb 14-23 took few weeks to study and lots of prayer and did the test last monday. just "trust in the lord with all your heart and lean not on your own understanding, in all your ways acknowledge him and he will direct your path":nurse::nurse:

I took the nclex RN last march 28 i did the pvt and i got the good pop up ....just wondering how long they usually post name on the CA BON ...just anxious to get the official result .. cant even tell my family the good news because its not the offial result yet...anybody took the nclex recently and saw there name right away in CA BON ?

hi i just wanted to share my stories as well i graduated in the Philippines way back 1997 for the last 13 years that i have been out of school i took nclex 3 x and unfortunately i failed my problem was being inconsistent of studying as you can see for almost 13 years i only did tried 3 x . i took the exam last august 2010 i had 265 question i hurried back home to check the PV trick and it took all the way CC page i was really upset and devastated ..i thought to myself that i will try it again in 3 months of course it didnt happened again ..i got busy working until January came i started studying . a friend of mine suggest to attend excell nursing review . im from the east cost and they dont have any schedule where i live. the last minute i decided to resign from my job and flew to san diego and take the 10 days review . its one of the best decision i ever made in my life . its a good comprehensive review especially i have been out of school for almost 14 years now ... so after the 10 days review i decided to stay in san diego with my family to study . i was going to take the test last march 12 but i re scheduled it because i felt i needed to do more studying... i have never studied like this my whole life 8-9 hours everyday in the library . and of course prayed a lot .... the day i took the exam which is yesterday i wrote in my scratch paper " Trust the Lord with all your heart and lean not into your understanding in all your ways acknowledge him and he will direct your path" and prayed the whole time i was answering question OMG i got a lot of SATA which im so bad about it. i probably had 15-20 of those i also got a arrange in order , lots of medication, priority and infection control my last question was a SATA at 75 i was pretty positive i got it right and computer shut off ... i started crying and i said myself i failed again.... 5 hours later i check the PVT and i got a good message. my anxiety levels reduced to at least 50% i just really hope that this PVT is accurate ..i have to wait for the official result CA BON .. i cant wait to see my name with an RN ... its like dreams comes true can u imagine after 14 yrs im finally becoming an RN... ill keep u guys posted...its never too late!

Hi everyone i took the exam today after studying for a long hours pretty much everyday for a long time im just glad its over . i graduated back 1997 and i took the nclex 3 x for the last 14 years last time i took it was august of last year i had 265 question i was pretty upset after i finished the test because i had the feeling i didn't do good well i did the PVT and it went all the way to credit card i was pretty upset and sure enough after 2 weeks i got the result in the mail i failed again the 3rd time... so needless to say i tried again i actually a friend of mine told me to try excell review and i actually have to fly to san diego and took the review for ten days . it was really good review being out of school for 14 yrs i really needed a comprehensive review i got it from them . after the review i gave myself a month to review i never studied so hard in my life like i did this time . i prayed a lot ....that god give me the knowledge , strength, wisdom to be able to pass the test. before i started the test this morning i wrote in my paper" "trust in the lord with all your heart and lean not on your own understanding, in all your ways acknowledge him and he will direct your path" the whole time i was taking the test i keep asking god to guide me to the correct answer .. when my computer shut off at 75 i cried...... i felt like OMG i didnt pass again.... so after 5 hours when i got home i tried PVT and i got the Good Pop! i was relieved a little bit. i know PVT is accurate i just cant wait to see my name in BON .. ill let you guys know how it goes....i hope its a good news ......

is this PVT is 100 % accurate?

Hi i took the exam yesterday , ill say it was different from my previous attempt. it was a lot of SATA at least 15-20 . i only got 1 calculation but a lot of priority and delegation and lots of infection control it took me at least 5 hours to answer all 265 question . i really dont know where i stand right now i have agood feeling about it though i just hope i did pass.. by the way how can i do the PVT ? do u actually register again and pay for it? please advice...
