I was just discussing this topic with some of my coworkers. Some nurses have observed new nurses giving IMs without z track and you could literally see the medication running out of their body as the needle retracted.

THere are a few nurses at my facility that worked as techs while in school, or while studying for boards...and then were offered nursing positions as soon as they passed!

We use those forms at my facility for q15min checks...they are coded with a letter for behavior, and number for location.
