Help with PKU Diagnosis
Ok so I know how to fix the deficient knowledge but I'm having issues with the delayed growth and development and it's kind of a big one with PKU. I understand what goes on and what I want to say but having trouble with putting it in NANDA form. PKU causes developmental delay and mental retardation due to a build up of phenylalanine in the blood and brain tissues when diagnosis is missed/delayed or treatment is delayed. Can you help me word this?

The assignment doesn't necessarily say that it has to be about and infant. I have picked some for different age groups and even the parents. Here is what I have so far, someone criticize me so I can fix any issues. Delayed growth and development r/t inability to break down excess amino acids AEB child's weight is in 5th percentile. Caregiver role strain r/t complexity of diet requirements AEB parent states not enough time to plan meals. Deficient knowledge r/t lack of experience AEB parent states she doesn't understand the disease process or the diet. Deficient fluid volume r/t excessive vomiting AEB dry mucous membranes. Non-compliance r/t adolescent's desire for increased independence AEB persistent elevated phenylalanine levels. I appreciate everyone's feedback. It really helps me with my paperwork. I have a really hard time trying to put what I want to say in NANDA terms.

Help with PKU Diagnosis
I am writing a paper on Phenylketonuria (PKU) and I have to come up with 5 potention NANDA's (No risk for's). I have come up with Deficient Knowledge r/t diet restrictions and Imbalanced Nutrition: less than body requirements (not sure of the r/t yet). Anyone know of anything else? I've never had a patient with PKU.

Help with understanding when to use hypo or hyper IV solutions.
Hypertonic solutions pull fluid from the tissues to reduce edema.

Help with DX
I have an OB patient and was going to go with Impaired Tissue Integrity but not sure if its tissue integrity or skin integrity. What do you think? I gave ice packs, tucks pads, epifoam, and instructed her on sitz baths to help with soreness and swelling. I was thinking skin integrity was more like preventing infections but not really sure.

Nursing Diagnosis for OB patient
I know that the perineal area is a good area to focus on with most patients however, this mom had little swelling, no bruising, didn't have any pain or want pain meds, no ice packs, no nothing. All I did to her this day was her postpartal assessment and get her things like ice water to drink and other non medical related things. I feel like I got the easiest patient in the world because she didn't want me to do anything. My care plan has to include three different assessments and interventions. She was great emotionally, VS were good, almost no lochia rubra present, fundus was good, breasts were good and baby was latching great. Great patient to have if you were the actual nurse in charge but not as a student looking for something to do. Am I missing something? I did a lot of teaching about what is going to happen to her body in the coming days.

Nursing Diagnosis for OB patient
I have an OB patient this is her third child and she has absolutely no emotional/medical issues. No history of medical issues nothing. I have to come up with a primary nursing diagnosis but it cannot be a risk for. Has to be about mom not baby. Was going for Effective breastfeeding but not sure what a long term and short term goal would be appropriate. Please help!
