  1. That's the problem...we do have a code blue sheet but instead, the ER doc writes the scenario on the order sheet. There was no legal document...
  2. I have this adult patient, in DNR and GCS 7/15 state, to be trans-out to regular room on MV from the ICU. The patient was endorsed to me intubated from ER with no ET size indicated but only the level 2 shifts prior to my shift. How can I identify the ET size if it was inserted already and the ER doctor, who intubated, is not present to seek for answer? I endorsed the said patient with an ASSUMED ET size for adult....I know it's wrong but I'm trying my best not to do it again. I'm still fresh from being an ICU nurse.

  6. SOUTHWESTERN UNIVERSITY :loveya: The school where I graduated. A year after, I'm planning to apply there because currently, I'm working in Asian Hospital and Medical Center. Working here in Manila is so costly and I have a family in Cebu waiting for me.
  7. I have been hired, as a probationary employee, in a telemetry unit. The work can be manageable but I urgently need your expert opinion on time managament. The MAIN ISSUE that I have with me, is the DOCUMENTATION process. With the MDs running around and wanting their orders to be carried out, and patients waiting to be attended, I have less time in DOCUMENTATION. We use APIE, which stands for A-Assessment, P-Planning, I-Implementation and E-Evaluation. Is there a need to make a long nurse's notes, with the unnecessary details to be included, such as: established rapport, initial assessment done, attended to their needs and queries made...IF, you apply them to all your patients? I mean, we do all of these tasks to our patients but do we have to document them one by one? My preceptor had told me that you have to jot down the things you have done, even the MDs that made their rounds, for legal purposes. So, I had to write the unnecessary details again and again to all my patients. I HAVE BEEN THINKING ABOUT FOCUS CHARTING, you document the things you did to intervene the observed problem. I URGENTLY REALLY NEED YOUR OPINION AND GUIDANCE!

  11. It's a good thing that I'm going to start my volunteer work this monday in a government hospital near our place! While working, I'll make sure to prepare for the interview!
  12. Wahhhh...I'm waiting for my initial interview for 1 month already. I heard this gossip that they freeze hiring male applicants!
  13. I heard that Asian Hospital is on manpooling status!

  15. Wahhh...maybe they stop freeze hiring, considering that they have to attend to those who have passed the application and the initial exam.
