I feel stupid. Can anyone answer a question about a care plan?
Hi there! I'm also doing my first few careplans and just posted a question for help....they're really difficult so far! Now, I am definitely not an expert on these but I can try to help you. Diaphoresis is increased sweating, so I guess to make a careplan you should address the reason for the increased moisture and your interventions should match that. Interventions could be frequent linen changes, temperature control, etc. Kinda hard to find the pathophysiology of such a vague symptom. Not sure if that was any help, I am sure someone much more experienced than I will give you more valuable information!! Good luck!!

This is only my second care plan ever, so bear with me. I need help formulating a nursing diagnosis for my patient. He is ESRD, Type II diabetic, CHF, Dementia patient, MRSA+ in his blood, admitted for fever and weakness. His GFR is 6, he is on dialysis 3x weekly. (At his last treatment they removed 800 ml fluid). Creatinine 9.3, BUN 90. Electrolytes all normal. He had extremely low urine output, 50 ml over an 8 hour shift. He's not oriented to anything except himself. He is unable to get out of bed due to weakness and pain in his left hip but can turn side to side. He has a good appetite and is eating normally (renal and ADA diet). Blood sugars all ok. He is neither dehydrated nor fluid volume excess (no edema, no crackles in lungs, etc.) I am leaning toward risk for impaired skin itegrity r/t decreased mobility, metabolic impairment and diabetes. Any other suggestions? I honestly don't know where to start!!

OMG, I can't believe it.....I got the call this morning, and I'M IN!!!!! I start August 18th!!!!!! You have no idea how unbelieveably excited, thrilled, and crazy happy I am right now!!!!! Thank you for your good thoughts and wishes...I'm over the moon!!!!!!

The only advice I can give you is to diversify yourself...don't just take pre-reqs for and plan only on LMC's RN program. Take the core pre-reqs first (Anatomy, micro, physio, english, math, psych) *and* nutrition (the LVN program requires it as well as other local ADN programs). Apply to the LVN AND the RN programs at LMC, and also apply to other area schools. Take the higher level math classes that are needed to apply to CSU East Bay. Apply to them all. Don't put all your eggs in one basket, because LMC is so hard to get into, and it's all luck. I don't want you to end up like me! (Applied 3 times, currently on the alternate list, don't know what's going to happen, and can't reapply next year because my sciences start to expire). Trust me, diversify!!

Los Medanos in Pittsburg. What they did was send out the letters to the accepted people (45) and the alternates. Then everyone had to send letters back either accepting or declining. 12 people outright declined, the others all confirmed. Then everyone had to take the TEAS test. The cut score on that is 67. If you don't reach the cut score, you lose your spot and an alternate gets it. Now, my hope is that 5 people either don't reach the cut score, or like you guys suggested, change their minds for some reason. Do people really do that? Accept the spot and then change at the last minute?? All I know is, the waiting is *KILLING* me. I called today and emailed the test administrator and everyone is telling me that they cannot give out any information and that I'll have to wait for a phone call. It's *KILLING* me, I tell you.

Bay area- jobs while waiting, pre-nursing school
I wouldn't waste the time and money getting your CNA. I've had mine for a year now and have been unable to find a job in a hospital in the Bay Area, and I've applied to every open CNA position I can find. I'm sure I could find a job at a SNF, but I really want to work in a hospital. Most hospitals want you to have 6 months or more experience before they will hire you as a CNA.

Well, I got my TEAS test done and over with, THANK GOD. That was nerve-wracking!! I did pretty well and I'm happy that I don't even have to think about it anymore....even better news, I found out they called alternate #12 today!!! Sooooo, that means ONLY 5 people have to fail for me to get in.....keep your fingers crossed for me!!!!!

Okay, so I talked to the admissions lady from the RN program at my school. I am an alternate (#17) for my ADN program. 45 students were chosen, and 20 alternates. All chosen students and alternates have to take the TEAS test and pass with 67 or they lose their spot. Being #17, I haven't had much hope thus far that there was any way I was going to get in. HOWEVER, I just found out that 11 of the 45 students chosen have declined their spot in the program, before the TEAS test. That means, hope of all hopes, that if just 6 people don't reach the cut score, I will get in!!!! I know it's terrible to wish for, but I HAVE to get in this year...I just HAVE to. What do you guys think? Do you think I have a chance at all, or should I just give up hope now?????? 6 of the 60 or so people have to fail...is that realistic???

I'm right there with you....I'm an alternate so I really don't know if I'm going to get into my program or not, but I know that I need to start thinking of plan B. My last year's plan B was the CNA thing but that really has not helped at all, lol! Seriously, I've had my CNA for a year now and after the millions of resumes/applications I've sent, I haven't received a single call back. And it was about $1500 for the program and 6 weeks that I wasted!! I'm not sure what I am going to do for plan B. Julz, the micro thing sounds awesome for you. You will work it out and in the end you'll look back and think, wow, this was exactly how it was supposed to go! Sometimes our best laid plans fail but give way to better and more exciting things later on....we just don't know it until we're there! Now, I've got to go start taking my own advice!!!!

I personally wouldn't recommend spending the time and money getting your CNA. I did that last year, and applied for every single hospital in my area...and have yet to even get a call back Really, nobody is hiring CNA's right now...even pre-nursing student CNAs, which is very discouraging. I'm sure you'd be able to find a job as a CNA at a SNF, but that's definitely not something I wanted to do.

Alternates???
I hope I get in too. I'm just not getting my hopes up. Unfortunately, the way the lottery is set up, there are no guarantees. Just because I was an alternate this year means nothing for next year...it's the same lottery, regardless. Which means, I could apply next year, for the fourth time, and not even get on the list at all. As it is, I can't even apply next year. My anatomy expires this fall, the fall semester anatomy class is already full, and next year my micro and physio will expire (there is a recency requirement for the sciences at my school). So, this is it for me. If they don't get to #17, I pretty much have to figure out something else to do with my life. *sigh*

Alternates???
LMC is the school where I applied. I'm #17 on the alternates list...not good odds, I think . This was my third time applying. They give the TEAS test to all the people who were accepted into the program, and the first 20 alternates as well. Whoever doesn't reach the cut score (67 composite) will get dropped from the program. I'm trying to be positive, but I really doubt 17 people out of the 45 who were picked are going to decide to either go elsewhere, or fail the TEAS. What do you guys think??

Hi everyone, I posted on another thread that I was chosen as an alternate in the ADN program for fall, in a lottery. I was wondering if anyone else was chosen as an alternate for any programs, and what you think the chances are of getting in? Any suggestions/advice appreciated!!
