what do you guys think of my working conditions?
If this is a true CCU (critical care unit/intensive care unit) then you are totally working in an unsafe work environment. The CCU/CICU/CVICU/MSICU that I've worked over the years are a 1-2:1 ratio, with only a 3:1 in a crunch. They are totally putting your license in jeopardy with a 3-4:1 pt ration (if this is considered ICU). CYA!!!

What would you do for this patient?
I'm not seeing anyone suggest a DNR

How frequent BP's on vasoactive meds?
Q15min or less if they're being titrated, Q30 or 60 if not being titrated (up or down) for past 12 hrs.

TLC removal,....what the highest,..
You've obviously never had to deal with a COPDer that gets a big, fat air embolism from the TLC removal at 20 degrees. The pt was having the TLC pulled before being discharged to home; went unresponsive, sent back to the ICU, where we had HAD her for the previous 2 weeks detoxing; died within 12 hrs. KEEP THE HOB DOWN and BEAR DOWN!! Thank goodness MY name wasn't on that chart! Lawsuit!

Highest BP you have ever seen on a pt

Highest PEEP you've seen?
28, last ditch effort, pt died anyway

Scrubs for tall women
I am 6'2 with a 36" inseam. I have found the Laundau tall has a good length. Tafford.com has a decent Cargo with 34.5" inseam, but with a long-crotch...you know that problem I'm sure. I also have a booty, so Dickey's did not work for me, nor did they seem to work for smaller, LONG-LEGGED friends of mine. MEDLINE states that they have a 36" inseam....THIS IS FALSE (Medline is one of those company-supplied scrub sets that they supply ORs with). I've never shopped here (basically due to the price), but http://www.cheap-scrubs.com/html/tall-scrubs.html states that they have 36" inseam that look pretty cute. Good luck!!!
