One thing i hate about starting as a new employee at a hospital, even as an RN with experience, is that everyone does things differently and preceptors you work with can be great examples or horrid role models. You just need to find you niche, but document to cover yourself, because when it comes down to it you are all by yourself during a deposition

I moved from a small hospital to a big one and from general floor to Burn ICU all in one jump, and I have not regretted it yet. It was scary, I was afraid other nurses might think I wasnt good enough or skilled enough or that I didnt have the experience i needed to be competent there. But it has been great, and even though I am still nervous about my new and more critical environment I am proueder of myself everyday, and not for just suviving, for loving it and flourishing. Take that leap, learn, and you will be happy with yourself and proud of what you can learn and do

We worked our butts off in school, and are the moment to moment decision makers on the floor. I have a great doc personally, but if i were admitted i would be looking at my nursing staff for the night, they are the ones making my care decisions, and it doesnt matter how great my doc is if he is not in house every second, which no one could expect. ITS ALL ABOUT THENURSES. I also work in a teaching hospital in the regional burn unit, I always have to brace myself when the new residents come in, so come july first my catch phrases will resemble, "Would you like to review that order?" "Would you like to give me a useful pain control regimen or would you like called in an hour?" and so on and so on

Some are skilled at giving report, and others tend to add alot of extra junk. I can tell if it is going to take forever depending on who was there that i have to listen to. We also use a kardex, makes it go a litle faster, and are having some who are skilled at report, transmitting iinfo you need for care in a efficient manner, meet with the whole unit and talk about how they do it and stuff, i wont stop the tape for anything other than a pt related note or an inturruption from a dr or situation that needs immediate attention

At our facility i was repremanded initially when a coworkers husband was hospitalized on our floor and she read his chart without him first giving permission. My LPN coworker noticed, stopped her and made a comment to the supervisor to get it on record in case an incident arose later(He was in for r/o viral meng. and they tested for syph.) as some things might be upsetting to her. An incident arose, and i would not allow her to view the chart again until he gave permission, but he was sleeping and she didnt want him distrubed so she made a complaint. At first i was in the hot seat but i stuck to my guns even when the she had the dr call and give me an order to let her read it i informed him the pt had to give permission first. After a big freakin ordeal i finally got a half hearted half apology and was vindicated, but with the emphasis they pretend they put on confidentiality it was upseting that it was so hard to get the facility to recognize the legal standards. I think a big reason for wanting the dr there is because some things that are regular assessment of dx terms may appear offensive to some laypeople who dont recognize their exact meaning or use, and that most of the time they would not be able to really understand much of the material. I agree with the comment about not having time to go over every detail in the middle of a shift i have had that experience with families who did get access to their family members chart

My daughter is only 18 months, but she knows what eewww yucks are. Unfortunetly it doesnt seem like most nurses are. I am always careful. What if I didnt wear gloves and took something home to my family? I am nervous about taking things home to them even when i am overly careful, which is most of the time. some nurses i work with dont, and i give reminders. My OBGYN draws blood sometimes in the office himself and NEVER wears gloves, it disgusts me, and i make a comment every time. The unfortunate thing about gloves is they aren't like steel toed boots, eventually you get a stick, even if your careful, accidents happen. The first time i did it was off of a needle that had just been used in a long time dialysis pt with vre of the urine and mrsa of the blood. talk about a scare, but i got lucky. I dont understand why people will not use them, if you dont like the powder, use powder free! I know there are problems with them, I have a latex allery, but they are our first and best defense for ourselves and our patient and families
