Moogie, I am currently working both places. It's the DON at the 2nd shift job were I am supervisor the ADON is there late everyday, she even said she don't mind holding the fort down a 1/2 hour cause she knows I don't get a lunch break if she doesn't. She didn't leave until 9:30pm last night doing paperwork. Achot Chavi, The Restorative position is flex, and I will be able to come in at 6:00am and leave at 2:30pm. I just wanted to work the hours for my week of orientation with the restorative nurse I was replacing. she was coming in strictly 9-5 despite flex time and I wanted to have as many hours with her as I could. but it worked out well I came in at 6am, did the quarterly assesments and careplans and she was able to go over what I did when she cam in and we went to the next step. She actually told me I got more completed in a day than she used to.

I recently interviewed and was offered a position as a restorative nurse for a 100 bed facility. After thinking about the offer several days, I decided to take the position. It is a step up to me. I have never done restorative before and they are willing to train someone from outside. It is more $$, flex time which allows me more time with my family. All positive right. Well when I went to tell my DON at the facility I currently work as second shift supervisor, she flipped out! She was upset that I even sent a resume out to another facility and tried to make me feel as if it was a poor choice to take a restorative position. Made comments that it was a waste of my time. Even hit me with the "why go to another nursing home, you should want to go to a hospital" line. I hate that statement when anyone says it but a DON!!!! Even had the administrator tell me that the facility I was going to won't give raises b/c they are not at full census. Any way, I took the position. I wanted to stay on and work both for a while as long as I can if they allowed me to come in 1/2 hour late. (facilities are 15 minutes apart) the ADON is ALWAYS there until 8pm or 9pm EVERY NIGHT. I thought to my self WOW when the administrator told the DON no, she wouldn't approve me to come in late. The other idea was to split the fulltime position with one of the weekend supervisors that was willing to pick up 3 days and I work the other 2. To this the administrator also said no, she wants consistancy on the shift. I understand that but the other supervisor works the second shift on the weekends! the days I'm off. I have never been late since I have been there. I am the only supervisor that HAS NEVER CALLED OFF,and 2 have only been there 3 months! I guess my question is am I being selfish or are they just being unreasonable???

I have also been told that if the resident you are "borrowing " from is on medicaid/medicare it can be viewed as medicaid/medicare fraud. Like VivaLasVegas stated, the ins. co paid for the meds with the intent for the designated resident to recieve it. I will get the med out of the EDK or call pharm and have it drop-shipped to the facility. Like dmb219 stated we are not allowed to put not available either, it is the "residents right" to have the meds available as ordered.

I know that at my facility the restorative aides and nurse help to feed residents. The charge nurses usually park the med cart near the entrance and get everyone as they go in the dinning area as to not pass meds in the dinning room, and they are in the area if needed.

Advice for newer RN to LTC supervisor position
My story is a little similar to yours. I started out on a Neuro ICU stepdown. Stayed about 4 months or so . I also worked PRN in LTC as a charge nurse and loved the LTC setting. After 6 months, I was offered a supervisor position which I turned down. I was offered the position 4 other times. Finally after a year I took it and love it. As far as advice, Be straight and fair across the board. No one is your friend. Not sure of the exerience the charge nurses you will work with have( I have 4/6 of mine new grad LPNs) dont do the thinking for them. have them use thier critical thinking skills, you are a resource.

I still like being in the clinical setting ,don't necessarily have to be "floor" nurse. Somewhere in the area of nurse educator, staff development. If that makes any sense.

Have a question, I am trying to decide which degree route to go. RN to BSN or this Bachelors in Health Care Administration LTC. I have heard from my co workers a mixed review only to realize they have little or no knowledge of the later. I am a ADN RN that is currently working as Nursing Supervisor at a LTC facility. I have also come across an opening for RN case manager position with the company I currently work for. The qualifications they are looking for are supervision experience and at least 2 years LTC, which I have both. Big question is, Which direction should I go???? I know that I don't want to continue floor nursing, but I love the clinical aspect of trouble shooting problems, initiating plans, measuring and documenting outcomes and teaching in my supervisor position. Any feed back appreciated.

Yes. Its a good place for new nurses. You learn a lot, have a lot of support from DON, ADON etc.

Courtesies to families
i know sometimes we as nurses and medical professionals have to take care of families in special situations. this was one of those times when it trully is an emergency. my sis- in -law was in hosp (post partum), we were visiting her and new niece, my mom started having chest pains. thankfully, those l&d nurses sprang into action getting vitals for her, called ed, and ecorted her to ed. but the ones who seek out attention because their wife or hubby is in hosp. go to minute clinic.
