SOC Oasis vs. ROC Oasis
Client is due for recertification and the RN does the Recert Oasis on 5/1, the start of the new cert. period would be 5/6, client is admitted to the hospital on 5/2. Client comes out on 5/22- do you do a SOC or ROC? Billing at my agency is telling me that when this person went in the hospital prior to the new cert. period...and stays in until after that new cert. date that it is a complete discharge and RN would have to do a SOC. What do you do?

Care Paths
To those of you using care paths vs. care plans...I am looking for any positive input in using these care paths compared to care plans. Especially in the chronic and Long Term Care clients.

Mo440
Thanks...we use Beacon Health also but our Adverse Outcome reports continue to show we have higher than the national level of wounds. We were wondering if MO440 was the reason. Traveler -- the HHRG is higher related to burns and trauma.

YES IT IS THAT DARN SKIN AND LESION QUESTION ON OASIS....MY QUESTION IS ARE ALL OF YOU NURSES ANSWERING THIS YES AS THE INSTRUCTIONS SAY THAT LESIONS ARE SCARS AND MOLES, ETC. AND EVERYONE I KNOW HAS A MOLE OR A SCAR. OR ARE YOU ONLY ANSWERING THIS YES IF THE CLIENT HAS AN OPEN WOUND? :balloons:

Mo245
THANKS FOR THE WEBSITE...CLEAR AS MUD IS RIGHT...BUT I THINK I HAVE IT NOW. CMS STATES DO NOT COMPLETE MO245 IF A V CODE HAS BEEN REPORTED IN PLACE OF DIAGNOSIS THAT IS NOT A CASE MIX DIAGNOSIS. THAT IS WHAT I WAS LOOKING FOR.

Mo245
When Answering Mo245 On Oasis The Rule Is Only Use It If A V-code Replaces A Casemix Diagnosis In Mo230-ortho, Diabetic, Neuro, Or Burns Trauma. I Read An Article From Beacon Health Stating To Also Put This Icd-9 Code In Mo240...so Your Secondary Diagnosis In M0240 Matches M0245. Is This The Way Your Agency Is Using V-codes? No One Is Coding Mo245 For Fitting Urinary Devices Or Anticoagulant Therapy Unless Your Secondary Diagnosis Was A Stroke...right?

HH Software Critique
we use clindoc through 3m-and interfaces with billing. always some little cliches but for the most part the nurses like it.

Home Care Weekend Coverage
I was wondering how does your agency cover weekend care? Especially, do your nurses take on call? At present we have one nurse who takes all calls except HHA changes and call ins. I work for a county agency and we also have to answer Prevent calls regarding rabies or west nile virus. The nurse takes on call for 1 week and may take it Q6wk. All nurses who have worked less than 5 years take call and the nurses who have worked longer can opt out. Currently we are in negoiations with management to make changes.

New Diabetic 1-3wk7 Old diabetic 2wk1,7wk7 - as a supervisor I would expect if there is no healing in a 2 wk period MD is contacted and wound care changed. We are using Saf-gel and duoderm Q3-7 D change to get away from expensive daily wound care under PPS. CVA 1wk1, 1-3wk7
