problem with question

A client who has pen order for on opioid analgesic request medication. The client last receive medication 8h ago. witch of these action should a nurse take prior to administrating of medication.

a) encourage client to try other pain relief measure first

b) determinate amount of medication client has received in past 24h

c)ask client to rate pain on scale 1-10

d) have the client use the bathroom
