At your facility, how often do you document on the Plan of Care? Every 24 hrs or every shift? Thanks!

    We're supposed to do with every change in caregiver, so it could be multiple times per day, but usually just twice.

    We do every shift.. But I work nights , so I have to sign off on them from 7-11p on the 7th and 11-7a going into the 8th..
