At 8:30 pm 23 year old male pt is complaining of inability to "take a leak" . Complains of abdominal pain and urge to go, but cannot. Lower abdomen is rounded and firm . Physician has order a straight catheter to be inserted and removal of 250 cubic centimeters of urine ,to be repeated in 1 hour as needed if no relief. Insertion of catheter results in 250 cubic centimeters of urine returned . Urine is cloudy and straw colored . At 9:45 pm patient still complaining of abdominal pain and urge to urinate . Abdomen still feels firm, and appears distended . You repeat procedure and again 250 cubic centimeters of cloudy straw colored urine is returned . Patient states " I feel much better now " . Lower abdomen is now flat

P.S. you wouldn't write "take a leak" in a note.You would say " pt c/o being unable to void".
