Hi Guys, I need help to prepare a care plan on a scenario I was given. It must include the assessment, goals, nursing intervention, rationale, evaluation. Following is the Scenario:
Scenario
Mark a 60-year-old male admitted into a rehabilitation unit post left hip replacement. Mark is complaining of pain and is reluctant to move out of bed, despite being encouraged by doctors and nurses. He also has arthritis on right hip and knee, which is impairing his ability to mobilise. He requires assistance with bathing.
1.Identify two actual nursing diagnoses and one potential nursing diagnosis relevant to Mark's condition?
2. Outline 3 nursing interventions for each nursing diagnosis identified
3. Discuss how analgesia is important in managing Mark's compliance.
4. Identify which allied team members should be involved in Mark's care and their specific role in helping Mark to regain independence.
So far my answers to that are the following but cannot find any more possibilities and how can I elaborate on it:
Diagnosis: Pain related to post-surgery
Goal is to reduce pain and make patient more comfortable
3 nursing interventions: Administer analgesia as prescribed
Encourage non-pharmacological intervention
Encourage pt to mobilise at tolerable level
The team involved will be the occupational therapist, physiotherapist.
Please, could you help me find the answers required for this scenario? I will be very grateful.

Okay, you're on the right track. What service would be responsible for "rehab" after a "hip surgery." Super vital for getting strength back up. Think about it. You may mention them and role in your paper. What is the function of a physiotherapist regarding this patient? That doesn't make sense to me.

What semester are you? What resource are you using for care plans?

Do you have a sparks and taylor care plan book? Its a life saver and has the diagnosis, interventions, rationales, risk factors and more. Also, I think your goals should be more specific and measurable like stating a deadline, etc.
