Need Help Regarding Filling up the Application form for CALI

Hi there my name is anne from the Philippines. This is the 1st time to apply for nclex and i need help. I have a concern regarding filling up the application for licensure by examination form for CALI. Here are my concerns:

1) Under the professional education do i still need to fill up the portion there called california non-graduates coz im kinda confuse if this portion refers to Philippine graduates as well or for only the states in the U.S.A

2)Under the address portion if I live in the Philippines and there is a portion in the application form asking for a state, what will i write? Is it n/a or manila?

3) Lastly in the form of request for transcript there is a portion of social security no. is this referring to the SSN in the Philippines or United States?

Also if other portions there are not applicable like the corpsemen and california LVN 30-unit option will i just leave it blank or n/a?

Regarding your q's:

1) You do not need to fill up this portion as it refers to U.S. non-California graduates;

2) I wrote National Capital Region (NCR), and it was accepted ( I just took the NCLEX last Oct.) You can do this if your residence is in NCR;

3) Leave the portion referring to SSN blank as it refers to U.S. SSN. You will encounter this in the fingerprint card, and if you do not have the US SSN, just leave it blank.

about your first question about" cali non grads" just leave it blank, don't check e.g the corpsmen, LVN etc, just leave it blank.

2nd question about " state" just leave it blank also put Manila( are you fr manila?) or the city where you are from in space provided for "City".

about 3rd Q this refers to US SSC if you have one already put it there don't put your philippine SSC.I

If the whole word fits, might as well write it. But IMO, Phils. is better than PH.

hi to all the SuperModerators on this thread.. i have read the primers on top of this thread.. May I ask (to all who posts in this thread) how did you go about in preparing your requirements to be passed to CABON?..

(1)Like with the Transcripts with RLE and Diploma, is it really certified true copy or the original copy with the school seal?.. :uhoh21:

(2)and is the Nursing Program Verification Form really included as one of the requirements?..[or it does not apply to all applicants]

(3)May I please have a copy of the written explanation you sent to CABON, so I may have a guide in writing? ??? [PLEASE post it with your message]

(4) For the confirmation card, where can I get the First Class U.S. Postage Stamp?

(5) For the fingerprint card, I have the Aliases and the Social Security No. blank, which is more appropriate for me to write, NONE or NOT APPLICABLE(N/A)?..And with the race, since only one letter fits the small box, what information should I print? [i am from the Philippines, by the way.] :smilecoffeecup:

(6)And lastly, how did you deal with the bank draft?..What steps did you take?..
