There is not enough information here to help you.

Vag delivery? C section? baby full term? Any issues with labor or delivery? Did she have an episiotomy? Gravid 4 para 1 what was the issue with previous pregnancies? breast feeding? Partner involved?

What does your assessment look like? I'm not familiar w/ "NCP"--does it mean nursing care plan? You can't make a nursing dx based on her medical dx. I mean not every postpartum woman gets an infection (risk for, yes), and not every woman has a lot of pain esp after a first delivery. Breast or bottlefeeding? Any issues with... Bonding? Emotions/feelings? Father involved? Extended family support? Sleep? Knowledge deficits?
