NCLEX question practice

I think preparing for NCLEX can be fun through group discussion. I am glad I found this forum at allnurses. This thread is to practice some good NCLEX questions so we all can learn through interaction.

Anyone has thoughts on this question?

A nurse is caring for a woman that just had a Normal delivery in the Delivery room an hour ago. The nurse understands that the patient is still at risk for Uterine atony at this stage. All of the following interventions should be included in the care plan of patient for detection of uterine atony except:

A. Checking for saturated perinel pads every shift

B. Palpating the fundus at frequent interval.

C. Weighing perineal pads once they are changed, noting the time it was changed and the saturation.

D. Checking vital signs frequently for signs of shock

Specializes in retired LTC.

And your thinking the answer is ....

Is the question C. You should do the another 3.

Man Im bad at Maternity! But I'm going with A only because I feel like the pads should be check more frequently.

If we are looking at uterine atony this is the failure of the uterus to contract. This can lead to vessels to bleed more freely... which leads to... hemmorhage. So we do need the other three fundal height checked, weighing pads to indicate amount of blood lost, and vital signs are always good to have :)

Im thinking the correct answer is A because you need to check the pads more often than every shift.
